BRFSS Questionnaires

BRFSS questionnaires are designed by a working group of state coordinators and CDC staff. Currently, the questionnaire has three parts:

1. the core component, consisting of the fixed core, rotating core, and emerging core,
2. optional modules, and
3. state-added questions. All state health departments must ask the core component questions without modification. States may use modules if they wish, with or without modifying the questions. States may also add questions that address local priorities. States take responsibility for pre-testing and analyzing the questions they add to the survey

The BRFSS Questionnaires for 1987-2008 are available in PDF file format. The size of each questionnaire PDF file is approximately 300-600 KB. You will need the free Acrobat Reader to view them.

Documents posted in PDF, Word, and other formats on the Department of Health Web site will be made available on request to users who are unable to download or view them.
