H1N1 (Swine Flu) Vaccine Supply and Distribution

The H1N1 vaccine is available and being shipped to public and private health care providers. Although there will be enough vaccine for everyone, supplies will be limited initially.

• Licensed health care providers interested in enrolling in the H1N1 Vaccine Program to administer publicly-supplied H1N1 vaccine should contact their local health agency for more information.

Inventory and Doses Administered Reporting Tools and Forms

• Online H1N1 Aggregate Doses Reporting. Link to the online tool to report required weekly number of H1N1 doses administered.
• H1N1 Daily Vaccine Doses Administered Hash Mark Log (Excel, 55 KB) Nov. 20, 2009. A paper tool to record each dose of H1N1 vaccine administered daily by simply putting a hash mark through each number on the form under the appropriate age category. At the end of the week, the doses marked off on the sheet can be counted and used to calculate weekly totals for reporting.
• Weekly Summary Report for H1N1 Doses Administered (Excel, 104 KB) Nov. 20, 2009. A tool to capture summary data from H1N1 Daily Doses Administered Log, or a provider-owned information system, and used to complete weekly doses administered reports.
• Providers Monthly Report of H1N1 Vaccine Usage (Excel, 52 KB) Nov. 20, 2009. A tool to support local health and providers in monitoring their H1N1 vaccine inventory and use, meeting audit requirements, and guarding against vaccine fraud and abuse. Local health agencies do not have to aggregate these reports and submit them to the Department of Health.
