This toolkit contains information and tools that support SNAP-Ed projects. Your contributions and suggestions are welcome.

Energize Your Life! Eat Healthy-Be Active is a statewide campaign promoting good nutrition and physical activity for the public. Free materials, including logos and posters, are available.

FY2012 Contractor Profiles (PDF, 167KB) is a who’s who list for SNAP-Ed contractors across the state. It includes a summary of projects and contact information for the current year.

DOH English Photo Release Form (WORD, 26KB) is required to share photos you submit.
