Cómo Hacer una Reclamación al Programa de WIC del Estado de Washington

De vez en cuando personas quieren hacer una reclamación al programa de WIC estatal. La reclamación puede ser acerca de:

• Personal de WIC en la clinica.
• Un cliente de WIC.
• Personal de la tienda.

Cómo Contactar al Programa de WIC Estatal

Puede hacerlo en cuatro métodos diferentes:

1. Podría llamar por teléfono al 1-800-841-1410 y marque extensión 3620. Puedes hablar con un personal de WIC. Puedes dejar un mensaje y no importa el tiempo del día. No necesita dejar su nombre.
2. Manda la forma de los sospechas de fraude y abuso (71KB) por correo electrónico al wicfraud@doh.wa.gov
3. Imprime esta forma. Llenarlo. Mándalo por correo al::
