Make a Complaint to the State WIC Program

Sometimes people want to make a complaint to the state WIC program. The complaint might be about:

• A WIC clinic staff.
• A WIC client.
• A grocery store employee.

How to contact WIC to make a complaint

You can tell WIC in four different ways:

1. Call us at 1-800-841-1410 extension 3620. You’ll be connected to the Customer Service Representative. This WIC staff person is available Monday through Friday, from 8 a.m. to 5 p.m. You can leave a message at any time. You do not have to give your name.
2. Email the WIC Complaint Form (82KB) to wicfraud@doh.wa.gov
3. Fill out the form and mail it to:
   Dept. of Health
   WA State WIC Nutrition Program
   Integrity Unit–Fraud
   PO Box 47886
   Olympia WA 98504
4. Fax the form to 360-236-3777.

Documents posted in PDF, Word, and other formats on the Department of Health Web site will be made available on request to users who are unable to download or view them.
