WSC5: Diabetes and Cardiovascular Disease Prevention

Twenty-eight primary care teams are enrolled in this 13-month evidence-based approach to change their systems of care to improve the outcomes of patients with diabetes or who are at risk for cardiovascular disease.

Sustaining Change Initiative

September 2005 - March 2007

The Sustaining Change Initiative will assist organizations who have been through the Washington State Collaborative to sustain and spread planned care through providing tools and a supportive interactive experience that builds skills, confidence, competencies and infrastructure to improve the care of all patients.
