Child
Profile is Washington State's Health Promotion and Immunization
Registry system giving you important health and safety
information you need for your child. Child Profile helps parents
and providers work together to make good health care decisions
for kids.
Parents of
all kids born in Washington State get the materials.
Kids who move into the state can be added to the system by
their health care provider or parents can
sign up directly.

Health
and safety information you can use
The Child
Profile Health Promotion system sends age-specific information to
parents of kids aged birth to six years in Washington State.
The materials contain information about immunizations, well-child check-ups, nutrition, growth, development, and safety.

Easy
access to your and your child's immunization record
The Child
Profile Immunization Registry is a computer system in Washington
that helps doctors and nurses keep track of the
immunizations you and your child received. The Immunization Registry is a tool
for providers to make sure kids get the right
immunizations at the right times. The Washington State
Department of Health administers the system.
The Child
Profile Immunization Registry can help you make sure your
child's immunizations are up-to-date. The Immunization Registry can also
give you immunization records through your child's
health care provider. It may have the dates of all
your child's vaccines that you can use for school, child care,
and camp entry. Ask your child's health care provider if they
are connected to the Immunization Registry.
Obtaining your or your child's immunization information
Most health care providers in
Washington use the Child Profile Immunization Registry. Please
check with your provider first to see if they can give you a
complete immunization record. They can print it from the
registry or from their own medical record system. If they're
unable to do so, you may request your or your child's
immunization record from the registry. Please note that the
registry system does not have complete immunization records for
all people. To get your record from the Child Profile
Immunization Registry, please fill out the
Authorization to Release form (PDF, 24KB). A
Spanish Authorization to Release form (PDF, 27KB) is also
available.

Once the signed release form
is received, Child Profile will mail, fax, or securely
e-mail the immunization record to you within five business
days. If you have any questions, please contact Child

How does Child
Profile work?
Child
Profile gets information from the public portion of the
birth certificate from the Center for Health Statistics. Your
child's date of birth triggers the Child Profile system to send
each material about 30 days before your child's next
recommended well-child visit. You will get a total of 17
mailings from the time of your child's birth until age six years.
A team of
health professionals develop and regularly revise
these
materials. A team of
parent reviewers and
professional reviewers also review the materials.
Also, a
Spanish review team
reviews all
Spanish materials.

not wish to have your child's immunizations entered into the
Immunization Registry, talk with your doctor or nurse. If they use the Child
Profile Immunization Registry, they can take steps to honor your
request.
