About the Program

The Washington State Diabetes Prevention and Control Program promotes social, environmental, policy, and systems approaches at state and community levels to reduce the burden of diabetes among Washington residents.

What Guides Us:

Launched in 2005, the Washington State Diabetes Plan outlines goals and activities to prevent, manage, and control diabetes.

Program Priorities:

  • Control ABCs: A1C blood glucose, Blood Pressure and Cholesterol.
  • Focus intervention and evaluation activities on best and promising practices.
  • Develop and maintain critical partnerships to implement the state plan.
  • Change systems to support all people with diabetes. Find additional resources to expand diabetes work.
  • Work closely with programs that address other chronic diseases Target people with pre-diabetes with primary prevention programs.
  • Target populations that are most at risk.
  • Develop program organizational capacity and standard operating procedures.
  • Use data about diabetes for program decision making.

Partners:

The Washington State Diabetes Prevention & Control Program works with many partners across the state to implement the strategies outlined in the State Diabetes Plan.
