2012-2013 Provider Flu Vaccine Pre-book Survey
We're gathering information from health care providers to help
prepare the 2012-2013 flu vaccine pre-book for the State Childhood
Vaccine Program. We're gathering data online in partnership with the
Washington Vaccine Association. The link is open until January 16,
2012. We faxed a notice about the process to all providers enrolled
in the State Childhood Vaccine Program in December 2011, and also
notified provider professional organizations. The process allows
providers to give us input on the amount and type of flu vaccine to
pre-book for the program. It doesn't guarantee specific products to
individual providers.
The person in the clinic who manages flu vaccine ordering and knows about flu vaccine use should provide the data.
They'll need their vaccine ordering PIN and information about how much childhood flu vaccine the clinic will need in the 2012-2013 flu season.
