Driving directions from The East Side:

1. From I-405 South, take West Valley Hwy. Exit. At the end of the exit ramp, turn right onto West Valley Hwy. (68th Ave. South).
2. At South 204th St. turn left.
3. Take the second right, signed Creekside Entrance.
4. The Dept. of Health is located on the second floor of the Creekside Three Bldg. (#20435).
