Walk-In Same Day Service

The Customer Service Counter is open to the public 8:00 a.m. - 4:30 p.m. Monday through Friday, except state holidays. For further information please call (360) 236-4300. Orders received after 4:00 PM may not be processed the same day.

What Do I Need to Bring?

Download and fill out the ordering form, and bring it to the counter at the address above. You can also get this form at the counter in the office and fill it out there.

How Much Does It Cost ?

Walk-in service: Fees for each certificate ordered: $20.00 US - (Make checks/money orders payable to Department of Health). We accept cash, checks, MasterCard and Visa.

Order By Telephone

What Do I Need To Order By Phone?

You must have the following information to order:

Credit card number.
Credit card expiration date.
Address certificate is to be sent to.
If you are requesting UPS Next Day Air delivery (for an additional fee) we must have a street address. UPS Next Day Air cannot deliver to a post office box.

What Number Do I Call?

Order by phone by calling (360) 236-4313. Hours of operation are Monday through Friday 9:00 am to 4:30 p.m.

What Does It Cost to Order By Phone?

Cost for Over the Phone: One time service charge ( $6.50 VitalChek and $5 expedite) plus $20 for each certificate. The total cost is $31.50 for one certificate and first class mail. Payable by Visa, Mastercard, Discover and American Express.

For Expedited Delivery you must add:

Express Mail, and additional $18.30 US
UPS Next Day Air, an additional $18.50 US
UPS AK, HI & PR, an additional $25.50 US
UPS Canada/Mexico, an additional $26.50 US
UPS International, an additional $37.50 US

It normally takes 2 to 3 weeks to receive a certified copy of a certificate.
