form, instructions, mailing container, and return address label. Only kits
supplied by PHL or local health jurisdiction will be tested.

Specimen must be in the laboratory within 30 hours after collection. Samples
are accepted Monday through Thursday 8 a.m. to 5 p.m. and Fridays 8 a.m. to 12
p.m. Water Bacteriology kits MUST be prepaid. The fee is $20.
