About the Immunization Information System

The Washington State Immunization Information System (IIS) is a statewide, lifetime immunization registry that keeps track of immunization records for people of all ages. It is a secure, Web-based tool for healthcare providers and schools. The Washington State Department of Health administers the system.

How the System Helps Parents

If your doctor or clinic uses the IIS, they can:

  • Keep track of your child's immunizations and who gave them, even if you move, change providers, or lose your immunization records.
  • See which vaccines your child needs at each visit.
  • Print an immunization record for when your child starts child care, school, camp, or whenever you need it.

The data in the IIS is confidential – we will only give immunization information to you or your child's doctor, nurse, or clinic to help protect the health of your child. Individuals or parents may get their or their child's immunization information directly from the system by sending in a signed Authorization to Release (PDF) form. A Spanish Authorization to Release (PDF) form is also available. Parents who want access to their family's immunization records and adults who want access to their own records can sign up for MyIR.

How the System Works

The IIS gets birth data from the public portion of your child's Washington State birth certificate. Then, healthcare providers who have a formal written agreement with the IIS add immunization information to the system.

The Child Profile Health Promotion System also uses the public information from the birth certificate to send a series of 17 age-specific mailings, from birth to age six. The mailings focus on health, development, safety, well-child visits, and immunizations. A parent can call WithinReach at 1-800-322-2588 to get off the mailing list.

Who Can See My Child's Records?

Healthcare providers (as defined by the Washington State Health Care Information Act, RCW 70.02) who need immunization information in order to provide healthcare to a client, can access the IIS to see immunization information. They can also record immunizations given to their patients.

Keeping Track of My Child's Immunizations

The best way to keep track of your child's immunizations is to get an immunization card from your doctor, nurse, or clinic. As part of the Child Profile Health Promotion mailings, you get a Lifetime Immunization Record Card (PDF) in the introductory packet. If you don't already have one, call the Family Health Hotline at 1-800-322-2588 to request a card. Take it to every checkup so it can be marked with the vaccines your child gets. Keep it in a safe place. The IIS also helps you keep track of immunizations. Check with your doctor or nurse to see if they participate in the system.
