Privacy Complaint Process

Filing a Privacy Complaint Against the DOH Metabolic Treatment Product Program

Clients of the DOH Metabolic Treatment Product Program or their personal representatives may file written complaints with the DOH HIPAA Privacy Official regarding violations of the HIPAA requirements governing the privacy policies, procedures and practices.

 

Please include the following information in your complaint:

Information About The Person Filing The Complaint

• Name
• The best way to contact you
  • Phone – Day/Evening
  • May we leave messages at the phone number above? Yes / No
  • Mailing Address
• Do you want to be informed of the outcome of the complaint? Yes / No
• Any special accommodations needed for communication about this complaint
   

Information about the person(s) whose privacy rights you feel were violated

• Name
• Complainant’s relationship to this person
   

Description of Privacy Violation

• Describe what happened, including dates and names of people involved or any other information that would be helpful in identifying people to interview or records to review.
• If you are providing any documents do you want us to return them to you?

The DOH HIPAA Privacy Official will:

• log the complaint
• investigate the complaint
• notify the complainant of the complaint disposition within 30 business days from completing the investigation.
