Privacy Complaint Process

If you feel an organization has violated your privacy rights you may file a complaint with that organization and with the US Health and Human Services Office for Civil Rights.

Complaints Against Other Organizations

Persons who are not clients of the DOH Metabolic Treatment Product Program, or their personal representatives, may file complaints about other HIPAA covered entities with US Health and Human Services Office for Civil Rights (OCR).

Complaints Against the DOH Metabolic Treatment Product Program

Clients of the DOH Metabolic Treatment Product Program, or their personal representatives, may file written complaints with the DOH HIPAA Privacy Official regarding violations of the HIPAA requirements governing the privacy policies, procedures and practices.

Clients of the DOH Metabolic Treatment Product Program, or their personal representatives, may also file complaints with the US Health and Human Services Office for Civil Rights (OCR).
