    These recommendations are taken from the Department of Health's 2000 Medication Errors Report and Recommendations to the Legislature. They were developed in consultation with the regulatory boards, ad commissions, professional associations and Washington state hospitals. 

  • Helping Patients Keep Track of Medications
    These steps can help your patients keep track of their medications. This approach has been used by many provider systems (such as the Premera Blue Cross system) with great success. The Department of Health is endorsing this medication record and making it available to you and your patients online. Just print it out and follow the plan.
