State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA ¾ The state Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers. The department regulates the practice of health professions and enforces health and safety laws to protect the public from negligent, incompetent, or illegal health care practices. Consumers who think a health care provider acted unprofessionally are encouraged to file a report.

By calling the Health Professions Quality Assurance Division at the state Department of Health, people can get the disciplinary history, license status, and number of complaints against any credentialed health care professional in the state.

To place a complaint against a health care provider or get disciplinary information about a doctor, dentist, or other provider, call the Health Professions Quality Assurance Division at (360) 236-4701.

The state Department of Health is looking for public-spirited people to serve on regulatory boards, commissions, and committees. Benton County
In March 2003, the Pharmacy Board revoked the shopkeeper registration of Hak Chong Yim, doing business as Yim’s Food Mart (UBI601410338), for ten years. Yim must also pay a $10,000 fine. He knowingly sold excess quantities of a medication used in the manufacture of methamphetamine. Yim’s Food Mart is also known as River’s Edge Food Mart.

King County
In March 2003, the Nursing Commission suspended the license of Judith Taylor, a licensed practical nurse (LP36367), for three years. On several occasions, Taylor incorrectly transcribed and/or failed to transcribe a physician’s order for patients’ prescriptions.

Kitsap County
In March 2003, the Pharmacy Board revoked shopkeeper registration of Sung Kyu Lee, doing business as Trenton Avenue Grocery (UBI602070796), for 10 years. He must also pay a $10,000 fine. Lee sold medications in excess amounts and was criminally charged for the illegal sales.

Lewis County
In March 2003, the Nursing Assistant Program revoked the license of nursing assistant Rebecca Mullis (NA143683), with no right to petition for reinstatement for three years. The program also denied her application for a certified nursing license. She may not reapply for three years. On her application, Mullis concealed convictions of criminal trespass and driving under the influence.

Mason County
In March 2003, the Nursing Assistant Program indefinitely suspended the certification of Lori J. Buck, a nursing assistant (NC10064141), with no right to reapply for one year. Buck failed to respond to charges of unprofessional conduct for not complying with the requirements of a prior order.

In March 2003, the Nursing Assistant Program revoked the certification of Jeffrey L. Tinnerstet, a nursing assistant (NC10036026), for two years. Tinnerstet failed to respond to charges of unprofessional conduct for pleading guilty to fraud and unlawful issuance of a bankcard.

Skagit County
In March 2003, the Nursing Assistant Program revoked the registration of Cathy A. Short, a nursing assistant (NA135539), for five years. Short failed to respond to charges of unprofessional conduct for misrepresenting herself as a licensed practical nurse by submitting an employment application and resume stating she holds that license.

Spokane County
In March 2003, the Nursing Commission revoked the license of Jessie Ryan, a registered nurse (RN71583), for five years. Charges against her include diverting controlled substances for her own use, testing positive for controlled substances and failing to respond to a Department of Health investigator.

Whatcom County
In March 2003, the Nursing Assistant Program revoked the certification of Amber R. Patrick, a nursing assistant (NC10068873), for five years. Patrick abused patients by placing lemon wedges in their mouths without permission and laughing at them.

Yakima County
In March 2003, the Nursing Commission indefinitely suspended the license of Elizabeth B. Jones, a licensed practical nurse (LP11473). Jones failed to comply with the terms and conditions of a prior order. Suspension of her license will continue until she complies with the prior order.

Out of State
(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

Oregon: In March 2003, the Medical Commission indefinitely suspended the license of John H. Duong-Tran, a physician and surgeon licensed in Washington (MD38869). In January 2002, the Oregon Board of Medical Examiners suspended his medical license.

Indiana: In March 2003, the Nursing Commission indefinitely suspended the license of Clifford A. Lee, a registered nurse licensed in Washington (RN131773). In February 2002, the Kentucky Board of Nursing denied reinstatement of his license, but permitted his license be placed on probation after he met certain conditions.
