State disciplines health care providers
OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has taken disciplinary actions or has withdrawn charges against health care providers in the state.
HPQA is responsible for protecting and enhancing the health of the people of
Washington State by assuring access to safe, competent health care providers.
HPQA personnel work in partnership with 12 boards, four commissions, and nine
advisory committees to set licensing standards for 55 health care professions
(e.g., medical doctors, nurses, counselors).
Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/providercredentialsearch/). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Benton County
In June 2003, the Nursing Commission placed the license of Julie Gibbs, a licensed practical nurse (LP00041372) on probation for five years. She must pay a $250 fine and continue to comply with a monitoring program. Gibbs failed to comply with a prior order.
In July 2003, the Nursing Commission withdrew charges of unprofessional conduct against Marcia Wade-Hausenbuiller, a registered nurse (RN00044389).
Clark County
In July 2003, the Nursing Commission charged registered nurse Sara J. Smith (RN00139946) with unprofessional conduct. Smith allegedly diverted controlled substances that were intended for patients.
In July 2003, the Dental Commission issued a Final Order with dentist Steven J. Sorensen (DE00004256). He must pay a $5,000 to the commission and reimburse $2,300 to a patient’s parents. Sorensen provided orthodontic care that worsened rather than improved a patient’s teeth.
In July 2003, the Chiropractic Commission placed the license of chiropractor John W. Taylor (CH00003426) on probation for two years. Taylor must seek an evaluation, attend continuing education and pay a $3,500 fine. He had a sexual relationship with a patient and was convicted of Second Degree Assault.
In July 2003, the Nursing Assistant Program charged Melissa Ann Williams, an applicant for a nursing assistant registration (RA00026275), with unprofessional conduct. Williams allegedly concealed criminal convictions on her application.
Cowlitz County
In July 2003, the Nursing Commission reinstated the license of registered nurse Nikki LeDoux (RN00105874).
King County
In July 2003, the Massage Program charged David J. Boiselle, a massage therapist (MA00016348), with unprofessional conduct. In October 2001, Boiselle’s license to practice as a licensed practical nurse was indefinitely suspended. He also allegedly had sexual contact with a patient.
In July 2003, the Medical Commission amended a statement of charges against Theresa A. Burdick, a physician and surgeon (MD00016175). She is being charged with unprofessional conduct for allegations of failure to keep adequate documentation, failure to follow management protocol and failure to use an appropriate technique while treating a patient.
In July 2003, the Dental Commission charged dentist Thomas J. Laney (DE00005663) with unprofessional conduct. Laney allegedly failed to effectively manage a patient with an airway emergency.
In July 2003, the Chiropractic Commission placed the license of chiropractor Maagnus Morgan (CH00002170) on probation for one year. Morgan must pay a $2,000 fine. He failed to produce patient records in a timely manner.
In July 2003, the Health Care Assistant Program charged Heila M. Reed, an applicant for a health care assistant certification (CA00008391), with unprofessional conduct. Reed allegedly concealed a criminal conviction on her application.
Kitsap County
In July 2003, the Respiratory Care Program dismissed charges of unprofessional conduct against Michael R. Gruver, respiratory therapist (LR00001177).
Pierce County
In July 2003, the Board of Occupational Therapy Practice charged Shirley B. Kendall, an occupational therapy assistant (OC00000571), with unprofessional conduct. Allegations against Kendall include coming to work intoxicated and pleading guilty to negligent driving and driving under the influence.
In June 2003, the Nursing Commission entered into an Agreed Order with Linda D. Grant, a licensed practical nurse (LP00044494). Grant agrees to continue to comply with terms and conditions of a prior order.
Snohomish County
In July 2003, the Medical Commission placed the license of John C. Fina, a physician and surgeon (MD00033991) on stayed suspension. Fina must comply with the requirement of a monitoring program and pay a $2,000 fine. Fina misused controlled substances and wrote prescriptions for controlled substances for himself and a family member.
In July 2003, the Nursing Commission reinstated the license of registered nurse Lori J. Stach (RN00066362).
In June 2003, the Nursing Commission reinstated the license of Mike S. Weyerts, a licensed practical nurse (LP00045434). His license will be placed on probation for at least three years. He must complete a course and submit a report. Weyerts failed to communicate with his supervisor to clarify treatment of a patient, abandoned patient and failed to document medical procedures in a timely manner.
Whatcom County
In July 2003, the Medical Commission amended a statement of charges against David W. Drobnicki, a physician and surgeon (MD00024188). Allegations include sexual contact with a patient, failure to keep adequate patient records and pleading guilty to reckless endangerment and burglary.
In July 2003, the Medical Commission charged William E. Waltner, a physician and surgeon (MD00025496), with unprofessional conduct. Allegations against Waltner include prescribing medications to patients without adequate documentation, excessive prescribing of controlled substances and failure to address signs of addiction.
Yakima County
In July 2003, the Health Care Assistant Program dismissed charges against Henry A. McCormick, a health care assistant applicant (CA00007526).
In July 2003, the Medical Commission placed the license of Lila M. Stageberg, a physician and surgeon (MD00028147), on stayed suspension. She must join a monitoring program. Stageberg violated a contract with a monitoring program by obtaining a medication without a prescription and testing positive for narcotics.
Out of State
(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)
Oregon: In July 2003, the Dental Commission charged Phu T. Bui, a dentist licensed in Washington (DE00008339), with unprofessional conduct. Bui allegedly provided treatment recommendations to a patient that failed to meet the generally accepted standard of practice.
Nevada: In July 2003, the Medical Commission entered into an Agreed Order with Alexis Hallock, a physician and surgeon licensed in Washington (MD00036935). Hallock will allow annual compliance visits from the commission and pay a $2,500 fine. She pled guilty to reckless endangerment and illegal discharge of a firearm.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
