State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA - The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in the state.
HPQA is responsible for protecting and enhancing the health of the people of Washington State by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).
Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/hpqa1/Application/Credential_Search/profile.asp). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Grays Harbor County
In October 2003, the Department of Health suspended the certification of Mark David Thiroux, a nursing assistant (NC10037026), for failing to comply with a child support order.
King County
In November 2003 the Massage Therapy Program revoked the license of David J. Boiselle, a massage therapist (MA00016348), for 10 years. He had sexual contact with a patient and pled guilty to assault with sexual motivation.
In October 2003, the Medical Commission revoked the license of John A. Dippel, a physician and surgeon (MD00031163). Dippel is unable to practice with reasonable skill and safety due to mental condition.
In October 2003, the Massage Program and the Counselor Programs revoked the licenses of John D. Donnelly, a massage therapist and counselor (MA00015910, RC00043083), for five years. While performing massage therapy services, Donnelly had sexual contact with a patient.
In October 2003, the Nursing Commission indefinitely suspended the license of Christine L. Simmons, a licensed practical nurse (LP00044010). Simmons failed to comply with the terms and conditions of a prior order.
In November 2003 the Massage Program revoked the license of Curtis N. Wells, a massage therapist (MA00004439), for 10 years. He was found guilty of sexual contact with a patient.
Kitsap County
In November 2004 the Counselor Programs revoked the registration of Candace L. Collins, a registered counselor (RC00045252), for 10 years. Collins failed to comply with the terms and conditions of a prior stipulation to informal disposition.
Pierce County
In October 2003, the Department of Health suspended the certification of Ila Maria Waters, a nursing assistant (NC10044642), for failing to comply with a child support order.
Thurston County
In October 2003 the Nursing Commission indefinitely suspended the license of Elena M. Hunt, a registered nurse (RN00133562). Hunt failed to comply with the terms and conditions of a prior order.
Walla Walla County
In October 2003 the Counselor Programs revoked the registration of counselor Daniel L. Ferguson (RC00036170). He may not reapply for registration until his counseling license in California has been reinstated and he has completed ethics education. In 2001 the California Board of Behavioral Services revoked Ferguson’s California license to practice as a marriage and family therapist.
