State disciplines health care providers
OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has taken disciplinary actions or has withdrawn charges against health care providers in the state.
HPQA is responsible for protecting and enhancing the health of the people of
Washington State by assuring access to safe, competent health care providers.
HPQA personnel work in partnership with 12 boards, four commissions, and nine
advisory committees to set licensing standards for 55 health care professions
(e.g., medical doctors, nurses, counselors).
Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site ( https://fortress.wa.gov/doh/hpqa1/Application/Credential_Search/profile.asp ). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
There is currently an opening on the Orthotics and Prosthetics Advisory Committee for a licensed physician specializing in orthopedic medicine or surgery or physiatry. Please contact 360-236-4947 for information.
Benton County
In June 2004 the Nursing Commission charged Valerie A. Seabury, a
licensed practical nurse and registered nurse (LP00029681, RN00080263), with
unprofessional conduct. Seabury allegedly failed to comply with the terms and
conditions of a prior order.
Clark County
In June 2004 the Nursing Assistant Program charged nursing assistant
Barbara Jackson (NA00133838) with unprofessional conduct. Jackson allegedly
physically abused a patient.
Grant County
In June 2004 the Nursing Assistant Program charged nursing assistant
Joshua T. Shelton (NA00158143) with unprofessional conduct. Shelton was
allegedly observed smoking marijuana on a work break.
King County
In June 2004 the Pharmacy Board charged Valori Dauma, a pharmacy
technician (VA00019014), with unprofessional conduct. Dauma was allegedly
convicted of theft.
In June 2004 the Nursing Assistant Program withdrew a statement of charges against nursing assistant Mary W. Kamau (NC10066743).
In June 2004 the Counselor Programs charged Michelle D. Reynolds, a mental health counselor (LH00005289) with unprofessional conduct. Reynolds allegedly inappropriately crossed professional boundaries with a client.
In June 2004 the Nursing Assistant Program denied the application of Virginia A. Weldy to reactivate her license to practice as a nursing assistant (NA00128254). She may not reapply for five years. Weldy allegedly concealed she pled guilty to attempted forgery.
Lewis County
In June 2004 the Pharmacy Board denied the application of Teresa Scott
to practice as a pharmacy assistant (CA00054289). She may not reapply for five
years. Scott was convicted of assault and theft.
Okanogan County
In June 2004 the Pharmacy Board granted the request of Mid Valley
Hospital Pharmacy (HF00001018) for full reinstatement of its license to
operate as a pharmacy.
Pierce County
In June 2004 the Nursing Assistant Program charged nursing assistant
Gloria C. Corpuz (NA00142161) with unprofessional conduct. Corpuz was
allegedly convicted of unlawful issuance of bank checks.
In June 2004 the Counselor Programs placed the registration of Donal M. Kavanaugh, a registered counselor (RC00029625), on a stayed suspension for five years. He must pay a $1,000 fine, complete continuing education and be supervised for two years. Kavanaugh inappropriately crossed professional boundaries with a client.
In June 2004 the Counselor Programs charged James J. Mulhern, an applicant for a registration to practice as a counselor (CA00006562), with unprofessional conduct. Mulhern was allegedly convicted of arson.
In June 2004 the Dental Commission entered an Agreed Order with dentist Sanford G. Steinman (DE00004006). He agrees to complete continuing education, refund a patient and pay a $2,500 fine. Steinman provided treatment that failed to meet the generally accepted standard of care.
Skamania County
In June 2004 the Massage Program charged Michelle Runciman, licensed
to practice massage (MA00003215), with unprofessional conduct. Allegations
against her include requiring and/or encouraging unlicensed personnel to engage
in activities requiring licensure and hiring a person to perform massage who she
knew was not licensed.
Snohomish County
In June 2004 the Board of Osteopathic Medicine and Surgery placed the
license of Dennis S. Mann, an osteopathic physician and surgeon
(OP00000949), on a stayed suspension for two years. Mann must pay a $5,000 fine
and complete continuing education. He accepted gifts or informal loans from
patients, totaling $44,200.
In June 2004 the Health Care Assistant Program charged Dean F. Sutton, a health care assistant (HC00126255), with unprofessional conduct. Sutton allegedly abused a patient.
Spokane County
In June 2004 the Counselor Programs granted the application of Marc D.
Deavenport (CA00006155) to practice as a registered counselor. He must not
engage in solo practice, and must complete continuing education and pay a $1,000
fine ($500 is suspended). Deavenport was convicted of driving under the
influence, criminal impersonation, obstructing a law enforcement officer,
reckless driving and assault.
In June 2004 the Pharmacy Board granted the request of Stacie M. Hoover for reinstatement without conditions of her registration to practice as a pharmacy intern (IR00043175). Her request for permission to proceed with licensing to practice as a pharmacist was also granted. Hoover complied with a prior order.
Walla Walla County
In June 2004 the Nursing Assistant Program denied the application of Star
Hill to reactivate her license to practice as a nursing assistant
(NC10020814). She may not reapply for five years. Hill was found guilty of
forgery.
Whatcom County
In June 2004 the Nursing Commission placed the license of registered nurse
Clarice Kornelis (RN00052323) on probation for two years. She must pay a
$1,000 fine, undergo an evaluation, submit performance reports and may be
employed on a limited basis. The terms a prior order have been modified.
Yakima County
In June 2004 the Dental Commission modified an Agreed Order with dentist
Florian Thompson (DE00007249). He may perform root canals under 50 percent
supervision. All other terms and conditions of the prior order remain in effect.
In June 2004 the Nursing Assistant Program denied the application of Forrest Keeney to practice as a nursing assistant (CA00035367). He may not reapply for five years. Keeney concealed theft convictions on his application.
In June 2004 the Health Care Assistant Program charged Veronica A. Pacheco, an applicant to practice as a health care assistant (CA00009297), with unprofessional conduct. Pacheco allegedly was convicted of theft and driving under the influence and failed to cooperate with the investigation.
Out of State
(Many health care providers hold licenses in more than one state. Department
of Health receives information each month from national groups on disciplined
health care providers. When another state’s regulatory group takes actions, the
equivalent board/commission/program in Washington files unprofessional conduct
charges against the provider’s Washington credential.)
Idaho: In June 2004 the Nursing Commission placed the license of registered nurse John D. Bartosh (RN00136194) on probation for five years. He failed to accurately document administration of controlled substances and/or diverted controlled substances for his own use. He also repeatedly exceeded the physician’s order in the amount of administration and frequency of administration of patients’ medications.
Oregon: In June 2004 the Counselor Programs denied the application of Jennifer E. Sutton to practice as a registered counselor in Washington (CA00006220). She can reapply when she cooperates with an investigation. Sutton was convicted of burglary, assault and malicious mischief.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
