State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in the state.
HPQA is responsible for protecting and enhancing the health of the people of
Washington State by assuring access to safe, competent health care providers.
HPQA personnel work in partnership with 12 boards, four commissions, and nine
advisory committees to set licensing standards for 55 health care professions
(e.g., medical doctors, nurses, counselors).
Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site ( https://fortress.wa.gov/doh/hpqa1/Application/Credential_Search/profile.asp ). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
There is currently an opening on the Orthotics and Prosthetics Advisory Committee for a licensed physician specializing in orthopedic medicine or surgery or physiatry. Please contact 360-236-4947 for information.
King County
In June 2004 the Nursing Commission indefinitely suspended the licenses of
Jasbir Ahluwalia, a licensed practical nurse and registered nurse
(LP00029986, RN00105859). She failed to comply with the terms and conditions of
a prior order.
In July 2004 the Nursing Assistant Program revoked the certification of Polla S. Bauknight (NC10073701), a nursing assistant, for five years. She stole a patient’s credit card and used it to purchase items. Bauknight was arrested and charged with theft.
In June 2004 the Health Care Assistant Program revoked the certification of Bridgett Hamilton, a health care assistant (HC00131230), for seven years. Hamilton took a controlled substance from her workplace and had prescriptions filled that were not authorized by a physician. Bridgett Hamilton is also known as Bridgett Miller.
In July 2004 the Counselor Programs revoked the registration of Ellen E. Hendrick, a registered counselor (RC00018632). She failed to cooperate with an investigation.
In June 2004 the Counselor Programs revoked the license of Vicki L. Ruscigno, a mental health counselor (LH00006905), for five years. She inappropriately crossed professional boundaries and engaged in a relationship with a client.
In June 2004 the Unlicensed Practice Program issued an order against Patricia Styles to immediately cease and desist from the unlicensed practice of massage and/or nursing. She must also pay a $1,000 fine.
Kitsap County
In June 2004 the Nursing Assistant Program revoked the registration of
nursing assistant Lisa M. Jeffrey (NA00140734) for five years. Jeffrey
was convicted of theft.
Pierce County
In July 2004 the Nursing Commission suspended the license of registered
nurse Teresa A. McKay (RN00062048) for two years. She must also pay a
$250 fine. McKay was convicted of possessing a controlled substance.
Snohomish County
In June 2004 the Nursing Commission indefinitely suspended the licenses of
Christine Campbell, a registered nurse and advanced registered nurse
practitioner (RN00108728, AP30003392). Campbell failed to properly assess an
infant born with complications, and to treat, and transport the infant to a
hospital. The infant later died. She also failed to keep adequate documentation
on several patients.
In June 2004 the Counselor Programs revoked the registration and certification of Steven C. Harris, a registered counselor and chemical dependency professional (RC00017822, CP00001567), for 10 years. Harris was convicted of child molestation and failed to comply with a reporting requirement included in a prior order.
Spokane County
In June 2004 the Nursing Assistant Program revoked the certification of
nursing assistant Tracy Anne Luna (NC10066765) for 10 years. She signed
and cashed checks belonging to a patient without the patient’s permission. In
2003 the Department of Social and Health Services permanently placed her on the
OBRA Nurse Assistant Registry. Tracy Anne Luna is also known as Tracy Anne
Macedo-Arvizu.
Thurston County
In July 2004 the Nursing Assistant Program revoked the certification of
nursing assistant Susie Renea Towne (NC10075645) for seven years. Towne
was convicted of residential burglary.
Yakima County
In June 2004 the Medical Commission removed a stay from the suspended
license of physician Robert D. Bush (MD00020774). His license remains
suspended. Bush failed to comply with the terms and conditions of a prior order.
Out of State
(Many health care providers hold licenses in more than one state. Department
of Health receives information each month from national groups on disciplined
health care providers. When another state’s regulatory group takes actions, the
equivalent board/commission/program in Washington files unprofessional conduct
charges against the provider’s Washington credential.)
Arizona: In July 2004 the Nursing Commission suspended the license of Cherie Foley-Perdue, a registered nurse licensed in Washington (RN00138536). She must also pay a $500 fine. In July 2003 the Arizona Board of Nursing placed her license on probation.
