State disciplines health care providers

OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has taken disciplinary actions or has withdrawn charges against health care providers in the state.

HPQA is responsible for protecting and enhancing the health of the people of
Washington State by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (  https://fortress.wa.gov/doh/hpqa1/Application/Credential_Search/profile.asp ). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

There is currently an opening on the Orthotics and Prosthetics Advisory Committee for a licensed physician specializing in orthopedic medicine or surgery or physiatry. Please contact 360-236-4947 for information.

Chelan County
In June 2004 the Psychology Board placed the license of psychologist Glen A. Frese (PY00000990) on a stayed suspension for three years. He must reimburse patients amounts they paid to him for treatment. Before Frese is released from the stayed suspension, he must undergo an evaluation. Frese inappropriately touched patients and made inappropriate comments to patients.

In June 2004 the Pharmacy Board charged Laura D. Malliot, a pharmacy technician (VA00021909), with unprofessional conduct. Malliot allegedly diverted a controlled substance from her workplace and possessed a controlled substance without a valid prescription.

Franklin County
In June 2004 the Dental Commission charged dentist Harry T. Keyes (DE00002863) with unprofessional conduct. Keyes allegedly provided treatment that failed to meet the generally accepted standard and violated a prior order by practicing after he had agreed to cease practice.

King County
In July 2004 the Nursing Commission reinstated the license of Janice M. Bell to practice nursing (LP00035184).

In July 2004 the Nursing Assistant Program reinstated the registration of Allen F. Belz to practice as a nursing assistant (NC10051063).

In July 2004 the Counselor Programs granted the application of Michael A. Fishback to practice as a registered counselor (RC00035750). The suspension of a prior order has been stayed and he must comply with terms and conditions of that order.

In June 2004 the Counselor Programs placed the registration of Robert D. Lydon, a registered counselor (RC00027049,) on a stayed suspension. He must comply with terms and conditions of the order. In February 2004 Lydon’s certification to practice as a chemical dependency professional was suspended for three years.

In July 2004 the Dental Commission dismissed a statement of charges against Dzon M. Nguyen (DE00007675).

In July 2004 the Nursing Commission entered an Agreed Order with registered nurse Susan Marie Taplin (RN00116981). Taplin license remains on probation for two years. She must pay a $750 fine and be employed as a nurse only under terms and conditions. Taplin had a sexual relationship with a patient.

In July 2004 the Health Care Assistant Program granted Richard B. Torres a certificate to practice as a health care assistant (CA00008588). He must pay a $1,000 fine ($500 is stayed) and obtain training. Torres was convicted of assault and reckless endangerment.

Lewis County
In July 2004 the Counselor Programs denied the application of Randall D. Robinson to practice as a registered counselor (CA00006485). He may reapply once he complies with the Department of Health investigation. Robinson was convicted of bail jumping, taking a motor vehicle without permission and physical control.

Pierce County
In July 2004 the Nursing Commission charged Sheila Castillo, a licensed practical nurse (LP00048171), with unprofessional conduct. She was allegedly found guilty of theft.

In July 2004 the Nursing Assistant Program reinstated the registration of Frances M. Duckett to practice as a nursing assistant (NA00130379, RA00017292).

Skagit County
In July 2004, the Nursing Assistant Program denied the application of Keeley M. Bieber to practice as a nursing assistant (CA00036283). She may not reapply for five years. Bieber was allegedly convicted of theft.

In July 2004 the Nursing Assistant Program released Sonya D. Fortune from terms and conditions of a prior order. She may reapply for certification and registration to practice as a nursing assistant (NA00119504). Sonya D. Fortune is also known as Sonya D. Holland.

Snohomish County
In July 2004 the Nursing Assistant Program denied the application of Adrian Serrano to practice as a nursing assistant (CA00036245). He may not reapply for three years. Serrano allegedly pled guilty to domestic violence.

In July 2004 the Nursing Assistant Program charged nursing assistant Matthew G. Skews (NC10010443) with unprofessional conduct. Skews’ registered nursing license was placed on probation for five years after diverting controlled substances from his workplace.

In July 2004 the Nursing Commission charged registered nurse Judith Wangen (RN00086893) with unprofessional conduct. Allegations against her include failing to get approval before administering medications to patients and failing to appropriately document the administration or wastage of patients’ medications.

Spokane County
In July 2004 the Nursing Commission reinstated the license of Brenda L. Both (Desjardins) to practice as a registered nurse (RN00120214).

In June 2004 the Dental Commission dismissed charges against dentist Allan E. Hinkle (DE00003570).

In July 2004 the Nursing Commission charged registered nurse Paul L. Lanning (RN00086156) with unprofessional conduct. Lanning failed to comply with terms and conditions of a prior order and submitted specimens that tested positive for methamphetamine.

Whatcom County
In July 2004 the Nursing Assistant Program reinstated the registration of Monica J. Morris to practice as a nursing assistant (NC10054154).

Out of State
(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

Illinois: In July 2004 the Medical Commission charged Mark A. Mattos, a physician licensed in Washington (MD00040481), with unprofessional conduct. Mattos allegedly tested positive for cocaine.

Mississippi: In July 2004 the Medical Commission placed the license of April A. May, a physician assistant licensed in Washington (PA10004379), on a stayed suspension for an indefinite period. May must comply with terms and conditions. The action is based on disciplinary action taken in other states.

Texas: In July 2004 the Medical Commission charged Jerrold R. Albom, an applicant to practice as a physician in Washington (MC00015112), with unprofessional conduct. Allegations against Albom include admitting on a news program to traveling to Cambodia and paying to have sex with minors.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
