OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in the state.

HPQA is responsible for protecting and enhancing the health of the people of
Washington State by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/hpqa1/Application/Credential_Search/profile.asp). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

There is currently an opening on the Orthotics and Prosthetics Advisory Committee for a licensed physician specializing in orthopedic medicine or surgery or physiatry. Please contact 360-236-4947 for information.

Clark County
In July 2004 the Counselor Programs revoked the licenses of Homer S. Adams, a mental health counselor and a social worker (LH00004587, LW00007115). In September 2003 the Oregon State Board of Clinical Social Workers revoked his license to practice as a clinical social worker after findings of sexual misconduct between him and two of his clients.

Grant County
In July 2004 the Pharmacy Board revoked the license of pharmacist Lucian H. Moore (PH00013725) for 10 years. Moore fraudulently created and/or filled numerous prescriptions for controlled substances for his own and/or his wife’s use. He also diverted controlled substances from his workplace.

King County
In July 2004 the Nursing Commission indefinitely suspended the license of Patricia A. Camacho, a licensed practical nurse (LP00038610). She failed to comply with the terms and conditions of a prior order.

In July 2004 the Nursing Assistant Program revoked the registration of nursing assistant Peter G. Kigano (NA00148088) for 10 years. He was convicted of unlawfully displaying a weapon, assault and theft.

In July 2004 the Health Care Assistant Program indefinitely suspended the certification of Tashanda R. Rodgers, a health care assistant (HC00135962). Rodgers failed to comply with the terms and conditions of a prior order.

Kitsap County
In July 2004 the Nursing Assistant Program indefinitely suspended the registration of nursing assistant Gale L. Klein (NA00132859). Klein physically abused a patient, a vulnerable adult.

Pierce County
In July 2004 the Nursing Commission indefinitely suspended the license of registered nurse Diana T. Gerken (RN00064500). Gerken failed to comply with the terms and conditions of a prior order. She also told the Commission she is giving up the practice of nursing and health care.

In July 2004 the Chiropractic Commission revoked the license of chiropractor Jeffrey L. Kantola (CH00002636) for 10 years. He failed to comply with the terms and conditions of a prior order.

In July 2004 the Denturist Program granted the application of David W. Kirk (DN00000124) to reactivate his license to practice as a denturist. His license is suspended until he complies with terms and conditions. Kirk’s license will then be placed on a stayed suspension for five years. Kirk’s criminal convictions include unlawfully possessing a controlled substance with intent to deliver, forgery and possessing stolen property.

In July 2004 the Nursing Commission indefinitely suspended the license of Peggy M. McNamara, a licensed practical nurse (LP00038859). She failed to comply with the terms and conditions of a prior order.

Spokane County
In July 2004 the Pharmacy Board revoked the registration of Aaron Pena, a pharmacy assistant (VB00051445), for two years. His application to practice as a pharmacy technician is also denied and he may not reapply for two years. Pena was convicted of reckless endangerment.

Thurston County
In July 2004 the Pharmacy Board entered an Agreed Order with Karin Robles, a pharmacy technician (VA00017511). She agrees to surrender her certification and not resume practice as a pharmacy technician. Robles received a controlled substance from a patient in exchange for paying for the patient’s prescription. She also admitted to smoking marijuana.

In July 2004 the Nursing Assistant Program revoked the certification of nursing assistant Richard Quichocho (NC10057085) for 10 years. He used a patient’s credit card without permission.

Whatcom County
In July 2004 the Nursing Commission suspended the license of registered nurse Jan Marie Wijnhamer (RN00101953) for two years. She must also pay a $500 fine. Wijnhamer used controlled substances for other than legitimate or therapeutic purposes and tested positive for cocaine.
