State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in the state.

HPQA is responsible for protecting and enhancing the health of the people of
Washington State by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/hpqa1/Application/Credential_Search/profile.asp). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

There is currently an opening on the Orthotics and Prosthetics Advisory Committee for a licensed physician specializing in orthopedic medicine or surgery or physiatry. Please contact 360-236-4947 for information.

Clallam County
In August 2004 the Nursing Assistant Program revoked the certification of nursing assistant Mona P. Hester (NC10072821) for 10 years. Hester physically abused a patient at a nursing home.

Grant County
In August 2004 the Nursing Assistant Program indefinitely suspended the registration of Joshua T. Shelton (NA00158143). Shelton was observed smoking marijuana on a work break.

King County
In August 2004 the Nursing Assistant Program suspended the certification of Budhi Y. Margiono, a nursing assistant (NC10066354), for five years. He was convicted of assault.

Kitsap County
In August 2004 the Nursing Commission indefinitely suspended the license of Emalee Vidal, a licensed practical nurse (LP00049792). She diverted controlled substances from her workplace for her own use and tested positive for controlled substances.

Lewis County
In August 2004 the Unlicensed Practice Program issued a Cease and Desist Order against Greg Blackwood. He represented himself as a physician who treated sleep disorders. He must immediately cease and desist from the unlicensed practice of medicine and pay a $4,000 fine.

In August 2004 the Unlicensed Practice Program issued a Cease and Desist Order against Aliec Simmons. She used a forged license to work as a licensed practical nurse. She must immediately cease and desist from the unlicensed practice of nursing and pay a $2,500 fine.

Whitman County
In July 2004 the Dental Commission suspended the license of dentist Alfred M. Fairbanks (DE00004076) for five years. All but 60 days of the suspension will be stayed. Fairbanks must also comply with various terms and conditions. He failed to comply with the terms and conditions of a prior order.

Out of State
(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

Oregon: In July 2004 the Dental Commission entered an Agreed Order with Rosemarie C. Calderon, an applicant to practice as a dentist in Washington (DE00008607). She is prohibited from renewing her license to practice in Washington until she complies with terms and conditions. Upon license renewal, her license will be placed on probation for two years. The Oregon Board of Dentistry suspended Calderon’s license after she billed for services not provided, failed to properly document care given to patients and performed unacceptable patient care.

California: In July 2004 the Dental Commission revoked the license of Mark C. Evans, a dentist licensed in Washington (DE00008904), for 10 years. In December 2002 the Dental Board of California revoked his license to practice dentistry in that state.

California: In August 2004 the Registered Counselor Program revoked the registration of Richelle L. Yarbrough, a registered counselor licensed in Washington (RC00042306), for 10 years. She crossed professional boundaries and had sexual relationships with clients.
