State disciplines health care providers

OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has taken disciplinary actions or has withdrawn charges against health care providers in the state.

HPQA is responsible for protecting and enhancing the health of the people of
Washington State by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/hpqa1/Application/Credential_Search/profile.asp). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

There is currently an opening on the Orthotics and Prosthetics Advisory Committee for a licensed physician specializing in orthopedic medicine or surgery or physiatry. Please contact 360-236-4947 for information.

Clark County
In August 2004 the Pharmacy Board charged pharmacy assistant Sinoun Seng (VB00051564) with unprofessional conduct. Seng allegedly failed to comply with the terms and conditions of a prior order.

Grant County
In August 2004 the Nursing Assistant Program charged nursing assistant Peggie Lea New (NC10013794) with unprofessional conduct. She allegedly tested positive for marijuana.

In August 2004 the Nursing Assistant Program denied the application of Sarah McGiboney to practice as a nursing assistant (CA00037088). She may not reapply for five years. McGiboney failed to disclose a theft conviction on her application.

Grays Harbor County
In August 2004 the Registered Counselor Program denied the application of Troy M. Regan to practice as a registered counselor (CA00005930). He may not reapply for one year. Regan failed to disclose criminal convictions on his application.

King County
In August 2004 the Nursing Assistant Program charged nursing assistant Erika C. Adams (NC10074957) with unprofessional conduct. Adams allegedly failed to comply with the terms and conditions of a prior order.

In August 2004 the Nursing Assistant Program charged nursing assistant Devon N. Bauer (NA00161049) with unprofessional conduct. Bauer allegedly failed to comply with the terms and conditions of a prior order.

In August 2004 the Nursing Assistant Program denied the application of Carrie Crane to practice as a nursing assistant (CA00036878). She may not reapply for three years. Crane pled guilty to attempted forgery.

In August 2004 the Medical Commission charged physician Robin Lee Downey (MD00025489) with unprofessional conduct for alleged violation of the uniform disciplinary act.

In August 2004 the Massage Program placed the license of Ricco A. Giancana, licensed to practice massage (MA00017359), on a stayed suspension indefinitely. He cannot practice in hotels or private residences and must be in a group practice. He failed to comply with the terms and conditions of a prior order.

In August 2004 the Dental Commission charged dentist Shohreh Khorsandi (DE00007812) with unprofessional conduct. He allegedly provided services to a patient that fell below the standard of care.

In August 2004 the Pharmacy Board charged Robert J. Powell, a pharmacy assistant (VB00052036), with unprofessional conduct. He allegedly diverted controlled substances from his workplace.

Lewis County
In August 2004 the Nursing Assistant denied the application of Jennifer K. Arthur to reactivate her license to practice as a nursing assistant (NA00108265). She may not reapply for five years. Arthur pled guilty to possessing methamphetamine.

Pierce County
In August 2004 the Nursing Assistant Program charged nursing assistant Wendy Billings-Keblbek (NC10072293) with unprofessional conduct. She allegedly failed to comply with the terms and conditions of a prior order.

In August 2004 the Nursing Commission charged registered nurse Kevin R. Davis (RN00122276) with unprofessional conduct. Davis allegedly failed to comply with the terms and conditions of a prior order.

In August 2004 the Nursing Commission withdrew a statement of charges against Catherine Luria, a registered nurse and advanced nurse practitioner (RN00058067, AP30000001).

In August 2004 the Medical Commission charged physician Maryanne McDonald (MD00020851) with unprofessional conduct. Allegations against McDonald include poor documentation, inappropriate use of staff, deceptive responses to government investigation and intervention and creating an unreasonable risk of harm to patients.

Snohomish County
In August 2004 the Nursing Commission charged registered nurse Sandy A. Forester (RN00072466) with unprofessional conduct. Forester allegedly concealed a material fact on his reinstatement request.

In August 2004 the Nursing Assistant Program denied the application of Camie Holden to practice as a nursing assistant (CA00034722). She may not reapply for five years. Holden failed to disclose a theft conviction on her application.

In August 2004 the Nursing Assistant Program charged nursing assistant Teresita M. Morse (NA00105291) with unprofessional conduct. Morse allegedly pled guilty to theft.

Spokane County
In August 2004 the Nursing Commission charged registered nurse Jeffery B. Alexander (RN00129397) with unprofessional conduct. He allegedly failed to comply with the terms and conditions of a prior order.

In August 2004 the Nursing Assistant Program charged nursing assistant Randy R. Ferguson (NC10048774) with unprofessional conduct. He has allegedly failed to comply with the terms and conditions of a prior order.

Stevens County
In August 2004 the Medical Commission amended a statement of charges against physician William Frank Catelli (MD00017690). He is being charged with unprofessional conduct for allegedly obtaining a controlled substance by writing a prescription, providing money to pay for the prescription and instructing an individual to return the prescription to him.

Whatcom County
In August 2004 the Nursing Assistant Program charged nursing assistant Shannon Paterson (NC10049219) with unprofessional conduct. She allegedly submitted a positive drug test.

Yakima County
In August 2004 the Nursing Assistant Program granted the application of Marilyn L. Olvera to practice as a nursing assistant (CA00036207). Her registration will be placed on probation for one year. Olvera must complete a course and submit quarterly declarations and performance evaluation reports from her employer. She failed to disclose a reckless endangerment conviction on her application.

Out of State
(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

Idaho: In August 2004 the Podiatric Medical Board amended a statement of charges against Stephen A. Isham, a doctor of podiatric medicine licensed in Washington (PO00000185). He is being charged with unprofessional conduct for allegedly performing unnecessary surgery on a patient.

Out of Country
Quebec:
In August 2004 the Nursing Commission reinstated the license of Mary S. Stevens to practice as a licensed practical nurse in Washington (LP00042585).

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
