State disciplines health care providers

OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has taken disciplinary actions or has withdrawn charges against health care providers in the state.

HPQA is responsible for protecting and enhancing the health of the people of
Washington State by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/hpqa1/Application/Credential_Search/profile.asp). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

There is currently an opening on the Orthotics and Prosthetics Advisory Committee for a licensed physician specializing in orthopedic medicine or surgery or physiatry. Please contact 360-236-4947 for information.

Chelan County

In August 2004 the Pharmacy Board charged pharmacist Robert E. Blane (PH00009735) with unprofessional conduct. Blane allegedly refused to talk to a patient regarding concerns about a misfilled prescription. He allegedly delegated and/or allowed a pharmacy technician to consult with the patient.

Grant County

In August 2004 the Pharmacy Board charged pharmacist William L. Wallace (PH00007636) with unprofessional conduct. Wallace allegedly refused to talk to a patient regarding concerns about a misfilled prescription. He allegedly delegated and/or allowed a pharmacy technician to consult with the patient.

King County

In August 2004 the Registered Counselor Program and the Chemical Dependency Professional Program placed the registration and certification of Natalie B. Banks, a registered counselor and chemical dependency professional (RC00019437, CP00000101), on a stayed suspension for three years. She must pay a $4,000 fine ($3,000 is suspended) and complete continuing education. Banks crossed professional boundaries with clients.

In August 2004 the Psychology Board placed the license of psychologist Nathan Brown (PY00001841) on probation for four years. He may not engage in clinical practice for thirty months. He must pay a $10,000 fine, reimburse a patient and undergo an evaluation. He must have a supervisor when he returns to clinical practice. Brown had an inappropriate relationship with a client.

In August 2004 the Nursing Commission placed the license of Jacqueline W. Poole, a licensed practical nurse (LP00052370), on a stayed suspension. She must comply with terms and conditions. Poole was convicted of bank fraud

In August 2004 the Mental Health Counselor Program placed the license of Michelle D. Reynolds, a mental health counselor (LH00005289), on probation for three years. She must pay a $1,000 fine, complete continuing education and be supervised while practicing counseling. Reynolds crossed professional boundaries with a client.

In September 2004 the Pharmacy Board reinstated the license of Robert W. Sloan to practice as a pharmacist (PH00009846).

In August 2004 the Registered Counselor Program charged Charles P. Woodson, applicant to reactivate his registration to practice as a registered counselor (RC00026336), with unprofessional conduct. Woodson was allegedly convicted of residential burglary.

Kitsap County

In August 2004 the Nursing Assistant Program reinstated the registration of Monica L. Bonafede to practice as a nursing assistant (CA00035132).

In August 2004 the Mental Health Counselor Program and the Registered Counselor Program charged Robert M. Neely, a mental health counselor and registered counselor (LH00004002, MH30003289, RC00017304), with unprofessional conduct. Allegations against Neely include having a relationship with a minor and having sexual contact with clients.

Lewis County

In August 2004 the Medical Commission charged physician assistant Pamela J. Hubner (PA10003488) with unprofessional conduct. Hubner allegedly failed to comply with an order.

Mason County

In August 2004 the Nursing Commission placed the license of registered nurse David Scott (RN00134729) on probation for 18 months. He must pay a $1,000 fine, complete coursework and submit a report to the commission. Scott failed to consult with a physician before administering a patient’s medication.

Okanogan County

In August 2004 the Nursing Commission reinstated the license of Patricia Devault to practice as a registered nurse (RN00092125).

Pierce County

In August 2004 the Medical Commission placed the license of physician Robert L. Caldwell (MD00032296) on probation for one year. He must submit copies of prescriptions to the Medical

Commission, pay a $1,000 fine and complete a course. Caldwell gave a patient signed but undated prescriptions.

In August 2004 the Nursing Commission placed the license of registered nurse Sharleen A. Conner (RN00106814) on probation for five years. Conner must pay a $1,000 fine and get an evaluation. She diverted controlled substances from her workplace, failed to comply with a monitoring contract and was arrested for driving under the influence.

In August 2004 the Nursing Assistant Program denied the application of Linda Thanh Nguyen, an applicant to practice as a nursing assistant (CA00037462). She may not reapply for five years. Nguyen allegedly plead guilty to assault, obstructing a law officer and unlawfully possessing a controlled substance.

Spokane County

In August 2004 the Nursing Commission charged Timothy Anderson, an applicant to reactivate his licensed practical nursing license (LP00042476), with unprofessional conduct. In 1999 he voluntarily surrendered his license to practice nursing in Idaho and his license has not been reinstated.

In August 2004 the Registered Counselor Program and the Nursing Assistant Program charged William S. Irving, a registered counselor and nursing assistant (RC00038432, NA00160424), with unprofessional conduct. Irving allegedly crossed professional boundaries and engaged in a sexual relationship with a client.

In August 2004 the Nursing Commission placed the license of Leslie A. Martinez, a licensed practical nurse (LP00040533), on probation for two years. She must pay a $2,500 fine and submit performance reports. She may practice as a nurse only upon compliance with terms and conditions. Martinez failed to comply with the terms and conditions of a prior order.

In August 2004 the Registered Counselor Program denied the application of Richard D. West to practice as a registered counselor (CA00006212). He was convicted of assault and malicious mischief.

Thurston County

In August 2004 the Medical Commission charged physician Sarah Scott Reade (MD00034256) with unprofessional conduct. Reade was allegedly arrested for driving under the influence. She also allegedly possessed a controlled substance that she didn’t have a prescription for.

Walla Walla County

In August 2004 the Registered Counselor Program charged Georgena L. Brown, an applicant to practice as a registered counselor (CA00006207), with unprofessional conduct. In October 2002 her license to practice as a registered nurse was revoked for five years.

Yakima County

In August 2004 the Nursing Assistant Program denied the application of Celia Galindo to practice as a nursing assistant (CA00036629). She may not reapply for 10 years. Galindo pled guilty to theft.

Out of State

(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

Colorado: In August 2004 the Medical Commission amended a statement of charges against Michael A. Spector, a physician licensed in Washington (MD00035831). He is being charged with unprofessional conduct for disciplinary action taken in another state.

Idaho: In August 2004 the Medical Commission entered into an Agreed Order with Robert M. Nelson, a physician licensed in Washington (MD00041325). Nelson agrees to voluntarily surrender his license to practice medicine in Washington.

Oregon: In August 2004 the Medical Commission charged Daniel Boniface Hutson, an applicant to practice as a physician assistant in Washington (PC00004356), with unprofessional conduct. In 2003 he surrendered his license to practice medicine in Oregon.

Utah: In August 2004 the Nursing Commission placed the license of Kimberly S. Baker, a licensed practical nurse in Washington (LP00051659), on probation for two years. She must also pay a $500 fine. Charges against Baker include administering discontinued medication to a patient and failing to document wastage of medication.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
