State disciplines health care providers

OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has taken disciplinary actions or has withdrawn charges against health care providers in the state.

HPQA is responsible for protecting and enhancing the health of the people of Washington State by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/hpqa1/Application/Credential_Search/profile.asp). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

There is currently an opening on the Orthotics and Prosthetics Advisory Committee for a licensed physician specializing in orthopedic medicine or surgery or physiatry. Please contact 360-236-4947 for information.

Chelan County

In September 2004 the Pharmacy Board indefinitely suspended the certification of Laura D. Malliot, a pharmacy technician (VA00021909). She diverted a controlled substance from her workplace and possessed a controlled substance without a valid prescription.

Clark County

In September 2004 the Department of Health indefinitely suspended the license of physician Rafik M. Hanna (MD00025023). Hanna failed to comply with a child support order.

In September 2004 the Pharmacy Board revoked the registration of Sinoun Seng, a pharmacy assistant (VB00051564), for 10 years. Seng failed to comply with the terms and conditions of a prior order.

Grant County

In September 2004 the Pharmacy Board revoked the certification of Valori Dauma, a pharmacy technician (VA00019014), for 10 years. Dauma was convicted of theft for taking over $10,000 from her workplace.

In September 2004 the Nursing Assistant Program indefinitely suspended the certification of nursing assistant Peggie Lea New (NC10013794). New tested positive for marijuana.

King County

In September 2004 the Nursing Assistant Program revoked the certification of nursing assistant Erika C. Adams (NC10074957) for five years. Adams failed to comply with the terms and conditions of a prior order.

In September 2004 the Unlicensed Practice Program issued a Cease and Desist Order against Abbi Bowers. She must immediately cease and desist from conduct constituting practice of radiologic and/or x-ray technology and must pay a fine. Abbi Bowers is also known as Abbi Cordero.

In September 2004 the Registered Counselor Program revoked the registration of counselor Theresa A. Jordan (RC00044262) for three years. She failed to comply with the terms and conditions of a prior order.

In September 2004 the Pharmacy Board suspended the certification of Terryl L. Smith, a pharmacy technician (VA00040898), for six months. Five months of the suspension is stayed and her certification will be placed on probation for two years. Smith must submit periodic declarations and pay a $1,000 fine. She diverted drugs from her workplace.

Pierce County

In August 2004 the Nursing Commission suspended the license of Sheila Castillo, a licensed practical nurse (LP00048171), for five years. She must also pay a $1,500 fine. Castillo used food stamps belonging to a patient to make grocery purchases. She was convicted of theft.

In September 2004 the Nursing Commission indefinitely suspended the license of registered nurse Kevin R. Davis (RN00122276). Davis failed to comply with the terms and conditions of a prior order and a monitoring contract.

In September 2004 the Department of Health indefinitely suspended the certification of nursing assistant Calvin D. Gill (NC10057986). Gill failed to comply with a child support order.

In September 2004 the Registered Counselor Program revoked the registration of counselor Shirley B. Kendall (RC00042802) for two years. Kendall was convicted of driving with a suspended license and driving under the influence. She is subject to conditions under an order issued by the Occupational Therapy Board.

In August 2004 the Nursing Commission suspended the license of registered nurse Carlotta Getzin Martinson (RN00103069) for five years. If she complies with terms and conditions, the suspension will be stayed and her license will be placed on probationary status. Martinson diverted controlled substances from her workplace and fraudulently filled prescriptions.

Skagit County

In September 2004 the Nursing Commission suspended the license of David T. Seamon, a licensed practical nurse and registered nurse (LP00048624, RN00134449). He must also pay a $2,000 fine. Seamon failed to comply with the terms and conditions of a prior order.

Snohomish County

In September 2004 the Registered Counselor Program and the Marriage and Family Therapy Program suspended the credentials of Kenneth A. Burr (RC00036615, LF00001980) until at least March 2007. Burr must undergo an evaluation. If it is determined he can practice with reasonable skill and safety, his suspension may be stayed and subject to various conditions. Burr engaged in a sexual relationship with a client.

In August 2004 the Nursing Commission suspended the license of Linda Kasner, a licensed practical nurse (LP00047051), for two years. Kasner used methamphetamine and marijuana on a weekly basis.

In September 2004 the Nursing Assistant Program revoked the registration of nursing assistant Teresita M. Morse (NA00105291) for five years. Morse pled guilty to theft.

Spokane County

In September 2004 the Medical Commission revoked the license of physician Richard W. Illes (MD00039162) for 10 years. He was convicted of first degree murder in the state of Pennsylvania and sentenced to life in prison without parole.

In August 2004 the Nursing Commission indefinitely suspended the license of registered nurse Judith Lynn Robinson (RN00083267). Robinson used controlled substances for non-therapeutic use.

Thurston County

In September 2004 the Pharmacy Board revoked the license of pharmacy technician Susan Beggs (VA00042942) for 10 years. She diverted controlled substances and legend drugs from her workplace.

In September 2004 the Pharmacy Board suspended the certification of pharmacy technician Katherine A. Warner (VA00042037) for three months. Following the suspension period, her certification will be placed on probation and she must comply with terms and conditions. Warner refilled a patient’s prescription without authorization from the prescribing physician. She then purchased the prescription and delivered it to the patient.

Yakima County

In August 2004 the Unlicensed Practice Program issued a Cease and Desist Order against Alejandro "Alex" Guzman. He was offering and performing massage to the public for a fee. Guzman must immediately stop all conduct constituting the practice of massage and must pay a fine.

In September 2004 the Pharmacy Board revoked the certification of Stacie R. Smith, a pharmacy technician (VA00051116), for 10 years. She diverted controlled substances, legend drugs and other store merchandise from her workplace.

 

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
