State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in the state.

HPQA is responsible for protecting and enhancing the health of the people of
Washington State by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/hpqa1/Application/Credential_Search/profile.asp). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

There is currently an opening on the Orthotics and Prosthetics Advisory Committee for a licensed physician specializing in orthopedic medicine or surgery or physiatry. Please contact 360-236-4947 for information.

Asotin County

In October 2004 the Pharmacy Board indefinitely suspended the license of pharmacist Virginia K. Streeby (PH00011788). She is subject to a prior order but is no longer practicing pharmacy and has no intention of doing so in the foreseeable future.

Cowlitz County

In October 2004 the Pharmacy Board placed the license of pharmacist Lori Harer (PH00011285) on suspension for two years. If she complies with certain conditions, the second year of suspension will be stayed and she will be placed on probation for five years. Harper diverted a legend drug and controlled substances from her workplace.

Island County

In October 2004 the Unlicensed Practice Program ordered Tracy Wolfe to immediately cease and desist from the unlicensed practice of dentistry. Wolfe constructed and supplied custom appliances for teeth whitening.

King County

In October 2004 the Medical Commission entered into an Agreed Order with physician Vimlesh Ahmad (MD00020660). She agrees to permanently surrender her license to practice medicine in Washington. Charges against her include submitting false and fraudulent health care claims. These include claims at a higher level of service than she provided and billing for services she didn’t perform and office visits when she hadn’t seen a patient.

In October 2004 the Nursing Commission indefinitely suspended the license of registered nurse Susan Todd Akin (RN00081476). She failed to comply with the terms and conditions of a prior order. Susan Todd Akin is also known as Susan Dopko Akin.

In October 2004 the Department of Health suspended Michael L. Barker from practicing as a certified nursing assistant (NC10039444). Barker failed to comply with a child support order.

In October 2004 the Unlicensed Practice Program entered an Agreed Order with Rebecca Buck. She agrees to immediately cease and desist performing duties that require a dispensing optician license and pay a $1,000 fine. She measured and fitted prescription eyewear when she was not licensed.

In October 2004 the Registered Counselors Program modified a prior order to indefinitely suspended the registration of registered counselor Richard Thomas Cline (RC00028769). Cline failed to comply with the terms and conditions of the prior order.

In October 2004 the Dental Commission suspended the license of dentist Robert D. Redfield (DE00004246). He must comply with the terms and conditions of the order prior to his license reinstatement. Redfield failed to comply with the terms and conditions of a prior order and provided services that failed to meet the accepted standard.

Kitsap County

In October 2004 the Mental Health Counselor Program and the Registered Counselor Program revoked the licenses and certifications of Robert M. Neely (LH00004002, MH30003289, RC00017304) for 10 years. Neely engaged in sexual contact with minors.

Pierce County

In October 2004 the Nursing Assistant Program revoked the certification of nursing assistant Debt Begallia (NC10047876) for three years. She physically abused a patient and made derogatory remarks about the patient in the patient’s presence.

In October 2004 the Pharmacy Board indefinitely suspended the certification of Tonni Fincham, a pharmacy technician (VA00012395). She failed to comply with the terms and conditions of a prior order.

In October 2004 the Nursing Commission indefinitely suspended the license of registered nurse Heather Harsh (RN00131334). She failed to document wastage of controlled substances and failed to cooperate with an investigation.

In October 2004 the Pharmacy Board indefinitely suspended the license of pharmacist Lynn Irwin (PH00011061). She consumed legend drugs and/or controlled substances that were prescribed for another person, consumed a controlled substance in a non-therapeutic manner and diverted a controlled substance from her workplace.

Snohomish County

In October 2004 the Chemical Dependency Professional Program revoked the certification of Kimberley M. Laycock, a chemical dependency professional (CP00001457), for 10 years. She had sexual contact with a patient and altered a urinalysis test for the patient.

In October 2004 the Pharmacy Board suspended the license of pharmacist Edward Matsuwaka (PH00014541) for 60 days. He must also pay a $2,000 fine and will be on probation for tow years subject to conditions. Matsuwaka dispensed prescriptions that were not issued for the sole use of a patient and/or were not issued in good faith for legitimate medical or therapeutic purposes and/or pursuant to a valid physician/patient relationship. He also dispensed controlled substances without written prescription.

In October 2004 the Nursing Commission indefinitely suspended the license of Michelle M. Myers, a licensed practical nurse (LP00050289). Myers failed to comply with the terms and conditions of a prior order.

Spokane County

In October 2004 the Nursing Assistant Program revoked the registration of nursing assistant Denise R. Wickham (NA00140069) for 10 years. She abused a vulnerable adult and was convicted of manslaughter.

Thurston County

In October 2004 the Pharmacy Board revoked the registration of Daniqua Hall, a pharmacy assistant (VB00047150), for 10 years. Hall diverted controlled substances from her workplace for her use and/or to sell to others.

In October 2004 the Pharmacy Board revoked the registration of Victoria L. Safford, a pharmacy assistant (VB00053607), for 10 years. She diverted a controlled substance from her workplace.

Whatcom County

In October 2004 the Nursing Assistant Program revoked the registration of nursing assistant Zachary J. Waters (NA00137713), for five years. Waters was convicted of unlawfully possessing a controlled substance with intent to deliver.

Yakima County

In October 2004 the Radiologic Technologist Program revoked the certification of radiologic technologist Michele J. Andreotti (RT00001999) for five years. She failed to comply with the terms and conditions of a prior order.

Out of State

(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

Idaho: In October 2004 the Radiologic Technology Program revoked the license of Jared W. Walker, a radiologic technologist licensed in Washington (RT00006009), for five years. He was convicted twice of indecent exposure in Idaho.
