State disciplines health care providers

OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has taken disciplinary actions or has withdrawn charges against health care providers in the state.

HPQA is responsible for protecting and enhancing the health of the people of Washington by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/providercredentialsearch/). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

There is currently an opening on the Orthotics and Prosthetics Advisory Committee for a licensed physician specializing in orthopedic medicine or surgery or physiatry. Please contact 360-236-4947 for information.

Chelan County

In October 2004 the Nursing Assistant Program charged nursing assistant Mayra Minerva Bolanos (NC10071183) with unprofessional conduct. She allegedly used her sister’s name and social security number on her application. She was also allegedly terminated from her workplace because she did not have a nursing assistant license in her own name and social security number. Mayra Minerva Bolanos is also known as Xochile Patricia Bolanos.

In November 2004 the Nursing Commission charged registered nurse Robert B. Mullins (RN00131044) with unprofessional conduct. Allegations against him include failing to meet the standard of practice while serving as the director of nursing.

Clallam County

In October 2004 the Dental Commission placed the license of dentist Robert F. Henry (DE00004934) on probation for one year. He must comply with terms and conditions, including paying a $10,000 fine, completing continuing education and passing the jurisprudence exam. Henry aided and abetted unlicensed practice and several deficiencies were found during an infection control inspection of his workplace.

In November 2004 the Registered Counselor Program denied the application of Gary C. Wilgocki to practice as a registered counselor (CA00006600). He was convicted of driving under the influence.

Island County

In November 2004 the Nursing Commission charged Dyanne D. Desdier, a registered nurse and advanced nurse practitioner (RN00085870, AP30001764), with unprofessional conduct. She allegedly failed to comply with the terms and conditions of a prior order.

King County

In October 2004 the Nursing Commission charged registered nurse Cynthia G. Alne (RN00116039) with unprofessional conduct. She allegedly failed to comply with the terms and conditions of a prior order.

In October 2004 the Dental Commission entered an Agreed Order with dentist Tina Hoang (DE00008580). She must pay a $5,000 fine, complete continuing education and permit an investigator to audit and review her practices. Hoang provided treatment that failed to meet the accepted standard.

In October 2004 the Health Care Assistant Program and the X-Ray Technician Program modified an Agreed Order with George C. Patterson, an x-ray technician and applicant to be a health care assistant (CA00007093, XT00000926). He agrees to complete the requirements set forth in a prior order within one year.

In October 2004 the Nursing Assistant Program placed the certification of nursing assistant Jalali Whitten (NC10049136) on probation for two months, and she must complete a course. Whitten was convicted of harassment.

Okanogan County

In November 2004 the Pharmacy Board charged pharmacist Robert W. Ulrich (PH00007574) with unprofessional conduct. He allegedly failed to comply with the terms and conditions of a prior order.

Pierce County

In October 2004 the Nursing Commission reinstated the license of Sheila Cruse to practice as a licensed practical nurse (LP00032550). Her license is subject to probation. Cruse violated a contract with a monitoring program.

In October 2004 the Chemical Dependency and Registered Counselor Programs charged William J. Lind, a chemical dependency practitioner and registered counselor (CP00000809, RC00031410), with unprofessional conduct. Allegations against Lind include failing to have several clients submit a required urinalysis.

In October 2004 the Registered Counselor Program charged Carl D. Rodin, a registered counselor applicant (CA00006936), with unprofessional conduct. He was allegedly convicted of unlawfully possession a controlled substance, assault and intimidating a public servant.

Snohomish County

In October 2004 the Nursing Commission granted the application of Larry Howard to reactivate his licensed practical nursing license (LP00040113). His license is placed on probation for at least three years. In September 2003 the Montana Nursing Board summarily suspended Howard’s licensed practical nursing license.

In October 2004 the Nursing Commission placed the license of registered nurse Judith Wangen (RN00086893) on probation for three years. Charges against her include diverting controlled substances from her workplace.

Spokane County

In November 2004 the Nursing Assistant Program reinstated the certification of Lisa K. Nelson to practice as a nursing assistant (NC10011043). Lisa K. Nelson is also known as Lisa K. Hendrickson and Lisa K. Medlen.

Thurston County

In October 2004 the Dental Commission withdrew a statement of charges against dentist Richard R. Busto (DE00005159).

In November 2004 the Nursing Commission reinstated the license of Karen L. Clanton to practice as a registered nurse (RN00050447).

In November 2004 the Nursing Commission charged registered nurse Katherine S. Scroggins (RN00108915) with unprofessional conduct. She allegedly failed to comply with the terms and conditions of a prior order.

Walla Walla County

In October 2004 the Nursing Assistant Program denied the application of Berta E. Abrego to practice as a nursing assistant (CA00036952). She may not reapply for five years. Abrego was convicted of theft and altering a license.

Yakima County

In October 2004 the Nursing Assistant Program charged nursing assistant Paula C. Garcia (NC10021034) with unprofessional conduct. She allegedly tested positive for methamphetamines.

Out of State

(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

Alaska: In November 2004 the Nursing Commission charged Greg L. Lane, a registered nurse licensed in Washington (RN00135313), with unprofessional conduct, based on charges taken in another state. Lane allegedly had action taken against his California nursing license.

California: In October 2004 the Nursing Commission reinstated the license of Shirlee Ann Driscoll to practice as a registered nurse in Washington (RN00039554). Shirlee Ann Driscoll is also known as Shirlee Ann Nickell.

Oregon: In October 2004 the Dental Commission placed the license of Donald E. Janoff, a dentist licensed in Washington (DE00004769), on probation for two years. His practice is restricted; he must complete continuing education, permit an investigator to audit records and review his practice, reimburse a patient and pay a $7,500 fine. Janoff provided treatment that fell below the standard of care.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
