State disciplines health care providers

OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has taken disciplinary actions or has withdrawn charges against health care providers in the state. The information on the cases reported in this news release represents the status of each item on the date the news release was issued.

HPQA is responsible for protecting and enhancing the health of the people of Washington by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/hpqa1/Application/Credential_Search/profile.asp). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Chelan County

In July 2005 the Licensed Mental Health Counselor Program charged mental health counselor Erik N. Bracht (LH00007519) with unprofessional conduct. Bracht allegedly developed a romantic relationship with a client.

In July of 2005 the Dental Quality Assurance commission amended a charge of unprofessional conduct against dentist Thomas K. Michael (DE00005736). Michael allegedly billed the Medical Assistance Administration for work he did not do, work that was unnecessary and work that did not successfully repair the teeth.

In July 2005 the Board of Pharmacy reinstated the license of Dale W. Murray to practice as a pharmacist (PH00011118). His license is reinstated without conditions.

Clark County

In July 2005 the Medical Commission entered an Agreed Order with physician Harry C.S. Park (MD00007018). Park agrees to surrender his license to practice as a physician. He failed to comply with a prior order.

Island County

In July 2005 the Board of Pharmacy placed the pharmacy technician credential of Maureen E. Wilson (VB00056269) on probation for three years. Wilson lied on her application regarding the earlier revocation of her nursing assistant certificate.

King County

In July 2005 the Board of Pharmacy reinstated the pharmacist license for Riel Dararak (PH00021399). The reinstatement was made without conditions.

In July 2005 the Board of Pharmacy reinstated the pharmacist license of Michael W. Dutton (PH00011668). The license was reinstated without restrictions.

In July 2005 the Nursing Assistant Program denied the application of Christopher G. Henderson to practice as a nursing assistant (CA00040009). He was convicted in King County District Court on a drug-related charge.

In June 2005 the Nursing Commission suspended the license of Admala S. Reddy, a licensed practical nurse (LP00053244), for 18 months. The suspension will be stayed and his license will be placed on probation if he complies with terms and conditions. He failed to respond to requests for assistance in the care of a patient, failed to provide follow up care, failed to chart care and failed to cooperate with the investigation. 

In July 2005 the Nursing Commission charge licensed practical nurse Helenita Yu Robb (LP00042205) with unprofessional conduct. She allegedly took a controlled substance from the disposal container for her personal use.

In July 2005 the Pharmacy Board denied the application of Katherine T. Ross to practice as a pharmacy assistant (CA00057674). She may not reapply for 10 years. Ross was convicted of possessing stolen property.

In July 2005 the Registered Counselor Program granted a certificate with restrictions to practice as a registered counselor to Amy L. Russell (CA0007357). Russell was found guilty in King County Superior Court of attempted possession of cocaine in 2001 and burglary and possession of stolen property in 2003.

In July 2005 the Dental Commission filed amended charges of unprofessional conduct against dentist Douglas A. Seymour (DE00006613). Seymour allegedly billed for work he had not done.

In July 2005 the Midwifery Program charged midwife Janine M. Walker (MW00000209) with unprofessional conduct. Walker allegedly failed to comply with previous conditions set on her license.

In June 2005 the Pharmacy Board charged Robert E. Walz, an applicant to practice as a pharmacy assistant (CA00059736), with unprofessional conduct. Walz was convicted of communicating with a minor for immoral purposes.

In July 2005 the Massage Program charged massage practitioner applicant Carla A. Warren (CA00007268) with unprofessional conduct. Warren had been convicted of two separate felonies prior to applying for her license.

Kittitas County

In June 2005 the Dental Commission amended a statement of charges against dentist Dean C. Breithaupt (DE00005818). He is being charged with unprofessional conduct for allegedly providing treatment that fell below the standard and failing to record prescriptions in a patient’s chart.

In July 2005 the Nursing Commission charged registered nurse Valerie L. Marjerrison (RN00146675) with unprofessional conduct. She allegedly diverted controlled substances from her workplace for her own non-therapeutic use.

Lewis County

In July 2005 the Nursing Commission charged Karyl R. Craig, an applicant to reactivate her licensed practical nursing license (LP00046309), with unprofessional conduct. Craig was convicted of assault and jumping bail.

Mason County

In June 2005 the Dental Commission amended a statement of charges against dentist Shirley O. Daniels (DE00007922). She is being charged with unprofessional conduct for allegedly providing treatment that failed to meet the standard.

Snohomish County

In July 2005 the Nursing Assistant Program charged nursing assistant Bernard L. Black (NC10080168) with unprofessional conduct. Allegations against him include using a lancet on a patient that he used on himself and practicing beyond the scope of practice.

In July 2005 the Nursing Commission charged registered nursing applicant Jessica L. Hanna (CA00060622) with unprofessional conduct. In 2004 she was convicted in King County Superior Court of second degree assault.

In July 2005 the Board of Pharmacy denied the pharmacy assistant application of Joseph LaRocque (CA00058660) for unprofessional conduct. LaRocque had been convicted of possession of marijuana and drug paraphernalia. He is unable to reapply for five years.

In July 2005 the Nursing Commission charged registered nurse Judith M. Wangen (RN00086893) with unprofessional conduct. She allegedly failed to comply with the terms and conditions of a prior order.

Whatcom County

In July 2005 the Registered Counselor Program charged registered counselor Kathyrn J. Faulkner (CA00007045) with unprofessional conduct. She was convicted in Whatcom Superior Court of unlawful possession of a controlled substance with intent to deliver.

Yakima County

In July 2005 the Nursing Assistant Program removed all terms and conditions previously placed on the nursing assistant registration of Barbara A. Yates (NA00166248). Yates complied with all terms and conditions during a one year probationary period.

In July 2005 the Registered Counselor Program charged registered counselor Dylan R. Aamodt (CA00007457) with unprofessional conduct. Aamodt was convicted in U.S. District Court of possession of cocaine with intent to distribute.

Out of State

(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

 

California: In July 2005 the Medical Commission amended a statement of charges against Kenneth G. Petty, a physician licensed in Washington (MD00033716). He is being charged with unprofessional conduct. His Missouri license was revoked and his Oklahoma license was suspended based on drug-related conduct.

 

California: In July 2005 the Nursing Commission reinstated the registered nurse credential for Sandra L. Nolden (RN00147071) for a probationary period of five years. The credential for Nolden, currently living in California, had previously been suspended after she tested positive for illegal drug use.

 

Florida: In July 2005 the Medical Commission charged Norman S. Cohen, a physician licensed in Washington (MD00026267), with unprofessional conduct. His Florida license is restricted.

 

Florida: In July 2005 the Radiologic Technologist Program denied the credential application of Carlos F. Perez (CA00003457) for five years for unprofessional conduct. Perez, currently living in Florida, declined to give details of past convictions.

 

Idaho: In July 2005 the Nursing Assistant Program indefinitely suspended the credential of nursing assistant Casandra K. Nordland (NC10072879) for unprofessional conduct. Nordland, now living in Idaho, was convicted in King County Superior Court of assault and malicious mischief.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
