State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in the state. The information on the cases reported in this news release represents the status of each item on the date the news release was issued.

HPQA is responsible for protecting and enhancing the health of the people of Washington by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/providercredentialsearch/). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

King County

In July 2005 the Chiropractic Commission suspended the license of chiropractor Thomas A. Goodwin (CH00001943) for three years. The first three months of the suspension are not stayed; the remainder of the suspension is stayed and Goodwin must comply with terms and conditions. Goodwin had sexual relationships with patients, provided treatment that failed to meet the standard of care and failed to document treatment provided.

In July 2005 the Nursing Commission suspended the license of registered nurse Jean Greig (RN00146404) for five years. He diverted controlled substances from his workplace.

In July 2005 the Nursing Commission indefinitely suspended the license of registered nurse Patrick I. Linville (RN00090292). He diverted controlled substances from his workplace.

In July 2005 the Nursing Assistant Program indefinitely suspended the registration of nursing assistant Mark R. Paschall (NA00133499). He tested positive for cocaine use.

Kitsap County

In July 2005 the Health Care Assistant Program indefinitely suspended the certification of David L. Cooper, a health care assistant (HC00143483). He failed to comply with prior terms and conditions placed against his certification.

Pierce County

In July 2005 the Chemical Dependency Professional and Registered Counselor Programs revoked the registration and credential of Mark D. Johnson, a chemical dependency professional and registered counselor (CP00003035, RC00011089), for three years. He failed to comply with an investigation regarding patient care.

Skagit County

In July 2005 the Respiratory Care Program indefinitely suspended the license of Edward A. Boone, a respiratory care practitioner (LR00000511). He failed to comply with the terms and conditions of a prior order.

In July 2005 the Health Care Assistant Program indefinitely suspended the certification of Lorraine K. Staniford, a health care assistant (HC00143484). She failed to comply with prior terms and conditions against her certification.

Snohomish County

In July 2005 the Nursing Assistant Registered Counselor Programs indefinitely suspended the registrations of Ian S. Colella, a nursing assistant and registered counselor (NA00143263, RC00049134). He presented forged prescriptions to pharmacies for controlled substances for himself.

In July 2005 the Nursing Commission extended the revocation against registered nurse Sandy A. Forester’s license (RN00072466) for five years. Forester petitioned for reinstatement and submitted a falsified report in support of his request. His license was originally revoked in 2000 based on a California Board order that revoked Forester’s California license after he sexually abused patients.

Spokane County

In July 2005 the Nursing Assistant Program revoked the registration of Christopher J. Twelves, a nursing assistant (NA00104051), for 10 years. He pled guilty to possessing child pornography.

Thurston County

In July 2005 the Medical Commission indefinitely suspended the license of physician John T. Neilson (MD00023060). He is unable to practice medicine safely due to a mental or physical condition.

Yakima County

In July 2005 the Nursing Assistant Program indefinitely suspended the certification of nursing assistant Elise A. Martinez (NC10066680). She tested positive for a controlled substance.

In July 2005 the Nursing Assistant Program indefinitely suspended the certification of nursing assistant Daisy Lee Medina (NC10062781). She tested positive for a controlled substance.

Out of State

(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

California: In July 2005 the Unlicensed Practice Program issued a Cease and Desist Order against Steve Sampson. He must immediately stop the unlicensed practice of veterinary medicine in Washington. Sampson sedated a horse and performed a dental procedure.

Oregon: In July 2005 the Nursing Assistant Program revoked the certification of Xander P. Hernandez, a nursing assistant licensed in Washington (NC10080477), for 10 years. He took money from a patient without consent.
