State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in the state. The information on the cases reported in this news release represents the status of each item on the date the news release was issued.

HPQA is responsible for protecting and enhancing the health of the people of Washington by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/hpqa1/Application/Credential_Search/profile.asp). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In August 2005 the Registered Counselor Program suspended the registration of registered counselor Connie J. Aimone (RC00039130). In 2004 her registered nursing license was suspended for four years.

In August 2005 the Nursing Commission indefinitely suspended the license of registered nurse Ronald A. Fagan (RN00114202). He failed to comply with the terms and conditions of a prior order.

Chelan County

In July 2005 the Nursing Commission suspended the license of registered nurse Mary T. Smith (RN00151528). She mishandled drugs in her workplace or diverted controlled substances from her workplace for her own non-therapeutic use.

Clallam County

In August 2005 the Nursing Assistant Program indefinitely suspended the certification of Darlene M. Burrow, a nursing assistant (NC10072949). She diverted controlled substances from her workplace for her own non-therapeutic use.

Grays Harbor County

In August 2005 the Nursing Commission suspended the licenses of Angela D. Ping, a licensed practical nurse and registered nurse (LP00047874, RN00143187), for six years. The suspension will be stayed after three years if she complies with terms and conditions. Ping failed to comply with the terms and conditions of a prior order.

King County

In August 2005 the Unlicensed Practice Program issued a Cease and Desist Order against Jack Arnold. He must immediately stop offering and treating medical conditions. While an employee and /or agent of Abundant HealthServices LLD, Arnold treated medical conditions for a fee. He holds no health care credential in Washington.

While an employee and /or agent of Abundant HealthServices LLD, Cote treated medical conditions for a fee. She holds no health care credential in Washington.

In August 2005 the Nursing Commission indefinitely suspended the license of registered nurse Susan L. Villarreal (RN00130628). She failed to comply with the terms and conditions of a prior order.

Kitsap County

In August 2005 the Nursing Assistant Program indefinitely suspended the certification of nursing assistant Douglas A. Lake (NC10062565). He failed to comply with the terms and conditions of a prior order.

Lincoln County

In July 2005 the Nursing Commission indefinitely suspended the license of registered nurse Carol Maclin Ziemer (RN00030299). She failed to verify blood given to a patient for a blood transfusion matched the patient’s blood type. The patient was given the wrong blood and died of a reaction to the blood transfusion.

Pacific County

In July 2005 the Nursing Assistant Program indefinitely suspended the certification of nursing assistant Kasandra L. Lofling (NC10076861). She failed to follow a patient’s skin treatment plan and failed to report the patient’s skin ulcers. The patient suffered injuries comparable to a burn.

Pierce County

In July 2005 the Nursing Commission indefinitely suspended the license of John E. Cooper, a licensed practical nurse (LP00036766). He failed to comply with the terms and conditions of a prior order.

In August 2005 the Department of Health suspended the credential of nursing assistant Calvin D. Gill (NC10057986). He failed to comply with a child support order.

In August 2005 the Department of Health suspended the certification of nursing assistant Tamara McCoy (NC10074496). She failed to comply with a child support order.

In August 2005 the Nursing Commission indefinitely suspended the license of registered nurse Constance Myers (RN00072803). She diverted a controlled substance from her workplace for her own non-therapeutic use.

Snohomish County

In August 2005 the Nursing Commission indefinitely suspended the license of registered nurse Judith M. Wangen (RN00086893). She failed to comply with the terms and conditions of a prior order.

Spokane County

In July 2005 the Chemical Dependency Professional and the Registered Counselor Programs revoked the credentials of Gordon W. Abild, a chemical dependency professional and registered counselor (CP00003417, RC00035638). Abild crossed professional boundaries and had sex with a client.

In August 2005 the Nursing Assistant Program revoked the registration of nursing assistant Jeffrey W. Hoffschneider (NA00098412), for 10 years. He failed to respond to and report a significant change in a patient’s medical condition, resulting in the patient’s hospitalization and death.

Thurston County

In August 2005 the Nursing Commission revoked the license of registered nurse Keith P. Schoening (RN00105104) for 10 years. He inappropriately touched a patient in a sexual manner and was criminally charged with rape and indecent liberties.

Walla Walla County

In August 2005 the Department of Health suspended the credential of nursing assistant Rex W. Miller (NC10054779). He failed to comply with a child support order.

Whatcom County

In July 2005 the Nursing Commission revoked the license of registered nurse Jan Marie Wijnhamer (RN00101953) for five years. She continued work as a registered nurse after her license was suspended in July 2004.

Yakima County

In July 2005 the Nursing Commission suspended the license of Susan Oakes, a licensed practical nurse (LP00043867), for two years. She identified herself as a registered nurse on several occasions and worked as a registered nurse.

Out of State

(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

Idaho: In August 2005 the Pharmacy Board suspended the license of Jason J. Jungert to practice as a pharmacist in Washington (PH00039062). He must comply with the terms and conditions of an Idaho order, which suspended his license for one year.
 

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
