State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in the state. The information on the cases reported in this news release represents the status of each item on the date the news release was issued.
HPQA is responsible for protecting and enhancing the health of the people of Washington by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).
Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/providercredentialsearch/). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Chelan County
In October 2005 the Nursing Assistant Program revoked the registration of
Lonnie I. Reese, a nursing assistant (NA00166301), for five years. She
practiced as a nursing assistant while impaired by alcohol.
Cowlitz County
In October 2005 the Nursing Commission indefinitely suspended the license of
Diane R. Washam, a licensed practical nurse (LP00048788). The Florida Board
of Nursing suspended her license in 2001.
King County
In September 2005 the Unlicensed Practice Program issued a Cease and Desist
Order against Marcy Rivas. She represented herself as a therapist and
provided counseling services that she was paid for. Her registered counselor
credential expired in 1996 and has never been renewed.
Mason County
In September 2005 the Registered Counselor Program revoked the registration of
William F. Bentley (RC00027558) for 10 years. Bentley submitted false
credentials to licensing agencies, forged licensing documents that were
displayed to clients and borrowed more than $194,000 from clients without
repaying them. Bentley has been convicted of forgery, theft, securities fraud,
money laundering and perjury.
Pierce County
In September 2005 the Nursing Commission indefinitely suspended the license of
registered nurse Rebecca Blackshaw (RN00093382). She is unable to
practice with reasonable skill and safety due to a physical or mental condition.
In October 2005 the Nursing Commission indefinitely suspended the license of
registered nurse Dana Lynn Ward (RN00116287). She diverted controlled
substances from her workplace for her use and provided nursing services while
impaired by drugs.
Skagit County
In September 2005 the Medical Commission indefinitely suspended the license of
physician William Frank Catelli (MD00017690). He wrote prescriptions for
himself, gave money to a coworker and asked the coworker to return the filled
prescription to him.
In September 2005 the Unlicensed Practice Program issued a Cease and Desist
Order against Yerda Yearsley. She must immediately stop engaging in
conduct constituting practice as an occupational therapist. She practiced while
her license was expired and gave her employer altered licenses with a falsified
expiration date.
Snohomish County
In September 2005 the Nursing Assistant Program indefinitely suspended the
certification of nursing assistant Matthew G. Skews (NC10010443). He
failed to comply with the terms and conditions of a prior order.
Spokane County
In September 2005 the Registered Counselor Program revoked the registration of
registered counselor Corey M. Bauer (RC00038238) for 10 years. Bauer
smoked marijuana with a client and engaged in sexual activities with the client.
Stevens County
In September 2005 the Pharmacy Board entered into an Agreed Order with Frank
R. McPherson, a pharmacy technician (VA00042432). He agrees to surrender his
certificate. McPherson presented a forged prescription for his dog at a
pharmacy.
Thurston County
In September 2005 the Unlicensed Practice Program issued a Cease and Desist
Order against Richard Bennett. He must immediately stop practicing
medicine and counseling. Bennett must also pay a $2000 fine. He is not licensed
to practice as a health care provider but operated a biofeedback center and
offered treatments and cures for various illnesses and conditions.
In October 2005 the Registered Counselor and Nursing Assistant Programs revoked
the registrations of Anthony Wayne Grant, a registered counselor and
nursing assistant (RC00039211, NA00122839), for 10 years. He was convicted of
communicating with a minor for immoral purposes.
In October 2005 the Nursing Commission indefinitely suspended the license of
Joshua N. Roberts, a registered nurse technician (NS00150065). He failed to
comply with prior limitations against his license.
Out of State
(Many health care providers hold licenses in more than one state. Department of
Health receives information each month from national groups on disciplined
health care providers. When another state’s regulatory group takes actions, the
equivalent board/commission/program in Washington files unprofessional conduct
charges against the provider’s Washington credential.)
North Dakota: In September 2005 the Nursing Commission suspended the
license of Petra Shute, a registered nurse licensed in Washington
(RN00139752) for two years. In July 2004 the North Dakota Board of Nursing
suspended Shute‘s nursing license in that state.
