State disciplines health care providers

OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has taken disciplinary actions or has withdrawn charges against health care providers in the state. The information on the cases reported in this news release represents the status of each item on the date the news release was issued.

HPQA is responsible for protecting and enhancing the health of the people of Washington by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).

Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site. The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In September 2005 the Pharmacy Board charged Tiffany Maxwell, a pharmacy technician (VA00020474), with unprofessional conduct. She allegedly diverted a controlled substance from her workplace for her use.

In October 2005 the Health Care Assistant Program granted the application of Kay B. Stone to practice as a health care assistant (CA00009116). Her certification is placed on a stayed suspension for two years and she must comply with terms and conditions. Stone concealed that she voluntarily surrendered her license to practice as a licensed practical nurse.

Clark County

In September 2005 the Dental Commission charged dentist George G. Blakeslee (DE00003787) with unprofessional conduct. He allegedly provided services that failed to meet the standard of care.

King County

In September 2005 the Pharmacy Board charged Erin E. Beck, a pharmacy technician (VA00044402), with unprofessional conduct. She allegedly diverted controlled substances from her workplace and fraudulently obtained controlled substances.

In September 2005 the Health Care Assistant Program corrected a statement of charges against Richard M. Beecham, an applicant to practice as a health care assistant (CA00010687). He is being charged with unprofessional conduct for concealing criminal convictions on his application. He was convicted of driving with a suspended or revoked license, burglary, hit and run and driving under the influence.

In September 2005 the Denturist Program charged denturist Ernie M. Hernandez (DN00000293) with unprofessional conduct. Allegations against him include providing services when his credential was expired, failing to provide adequate records when requested and providing services that were deficient.

In September 2005 the Pharmacy Board charged pharmacist Michael S. Jones (PH00010993) with unprofessional conduct. He allegedly failed to comply with the terms and conditions of a prior order.

Kittitas County

In September 2005 the Dental Commission entered into an Agreed Order with dentist Dean C. Breithaupt (DE00005818). He agrees to comply with terms and conditions, including paying a $2,500 fine and completing continuing education. Breithaupt provided services that failed to meet the accepted standard and failed to keep appropriate records.

In October 2005 the Nursing Assistant Program placed the certification of nursing assistant Lynda K. Carroll (NC10060167) on probation. She must comply with terms and conditions. Carroll was convicted of two counts of Medicaid false statement.

Pierce County

In October 20905 the Registered Counselor Program granted the application of Michele A. Benedict to practice as a registered counselor (CA00007574). Her registration is placed on probation for four years and she must comply with terms and conditions. Benedict was convicted of unlawful use of a building for drug purposes and unlawful possession of a controlled substance.

In September 2005 the Nursing Commission charged Lizabeth P. Chenette, a licensed practical nurse (LP00051890), with unprofessional conduct. She allegedly failed to comply with the terms and conditions of a prior order.

In October 2005 the Nursing Commission denied the application of Chay Lee to practice as a registered nurse (CA00057683). She may not reapply for one year. Lee lied about attending a college on her application.

Skagit County

In September 2005 the Unlicensed Practice Program issued a Notice of Intent to issue a Cease and Desist order against Tina M. Floe. She continued to practice massage after her license expired. In October 2005 the Unlicensed Practice Program issued a Cease and Desist Order against Tina M. Floe.

Spokane County

In September 2005 the Nursing Commission withdrew a statement of charges against Quibilah N. Person, a licensed practical nurse (LP00047459).

In October 2005 the Medical Commission reinstated the license of John R. Quinn to practice as a physician (MD00004754).

In October 2005 the Chemical Dependency Professional and Registered Counselor Programs placed the credentials of Elizabeth J. St. Clare (RC00004431, CP00002659) on a stayed suspension for three years. She must comply with terms and conditions. St. Clare operated a chemical dependency treatment facility. She required non-credentialed employees to provide counseling services, condoned inappropriate relationships between an employee and two patients and billed for services not provided. Elizabeth St. Clare is also known as Elizabeth J. Bailey-St. Clare.

In August 2005 the Dental Commission entered into an Agreed Order with dentist Patrick J. Stevens (DE00005806). His license will be reinstated if he complies with terms and conditions within one year. Stevens’s license is expired.

Whatcom County

In September 2005 the Nursing Commission granted the application of Lori Dublack to practice as a registered nurse (CA00060722). Her license is placed on probation for five years and she must comply with terms and conditions. In May 2005 the Indiana State Board of Nursing indefinitely suspended Dublack’s Indiana nursing license. Lori Dublack is also known as Lori Geisleman.

In September 2005 the Nursing Assistant Program charged nursing assistant Danita R. Gansler (NC10067037) with unprofessional conduct. Allegations against her include possessing a patient’s checkbook without authorization and failing to respond to investigations.

In September 2005 the Nursing Commission entered into an Agreed Order with Brandy M. Schneider, a licensed practical nurse (LP00052649). She must comply with terms and conditions. She was convicted of soliciting to possess a controlled substance.

Yakima County

In October the Nursing Assistant Program reinstated the registration of nursing assistant Marilyn L. Olvera (NC10088261).

Out of State

(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

California: In October 2005 the Medical Commission withdrew a statement of charges against Yvette I. Ficekova, an applicant for medical licensure in Washington (MC00013787).

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
