State disciplines health care providers
OLYMPIA ¾ The Health Professions Quality Assurance Office (HPQA) of the Washington State Department of Health has taken disciplinary actions or has withdrawn charges against health care providers in the state. The information on the cases reported in this news release represents the status of each item on the date the news release was issued.
HPQA is responsible for protecting and enhancing the health of the people of Washington by assuring access to safe, competent health care providers. HPQA personnel work in partnership with 12 boards, four commissions, and nine advisory committees to set licensing standards for 55 health care professions (e.g., medical doctors, nurses, counselors).
Information on Washington’s health care providers is now available online at our new Provider Credential Search Web site (https://fortress.wa.gov/doh/providercredentialsearch/). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling HPQA, 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.| Benton County | Mason County | Spokane County |

Benton County
In November 2005 the Nursing Assistant Program denied the application of
Brenda E. Martinez to practice as a nursing assistant (CA00041280). She may
not reapply for five years. Martinez was convicted of shoplifting.
Chelan County
In November 2005 the Examining Board of Psychology charged psychologist
Jeffrey J. Startzel (PY00002609) with unprofessional conduct. Allegations
against him include having a social and sexual relationship with a client and
failing to keep appropriate documentation.
Grant County
In November 2005 the Pharmacy Board charged Maria E. Alexander, a
pharmacy assistant (VA00020902), with unprofessional conduct. She allegedly
diverted controlled substances from her workplace for her own use.
King County
In November 2005 the Nursing Assistant Program denied the application of
Bettie A. Alexander to practice as a nursing assistant (CA00041649). She may
not reapply for five years. Alexander was convicted of assault, theft and
attempted unlawful possession of a controlled substance.
In November 2005 the Chiropractic Commission charged chiropractor Theodore J.
Clark (CH00002219) with unprofessional conduct. Allegations against him
include several record keeping violations.
In November 2005 the Massage Program charged Sonja M. Gleason, an
applicant to practice massage (CA00007687), with unprofessional conduct. She has
several criminal convictions, including failure to return to a state work
release facility, possessing stolen property and unlawful possession of a
controlled substance.
In November 2005 the Nursing Commission reinstated the license of Phyllis E.
Hulslander to practice as a licensed practical nurse (LP00036462).
In November 2005 the Nursing Commission reinstated the license of Karen Monti
to practice as a registered nurse (RN00082245).
In November 2005 the Nursing Assistant Program reinstated the certification of
Atta Panyin Serebour to practice as a nursing assistant (NC10047435).
In November 2005 the Medical Commission amended a statement of charges against
physician Richard Toby Sutcliffe (MD00020643). He is being charged with
unprofessional conduct for conviction of several counts of indecent liberties
and assault. The Court’s sentence includes permanent revocation of his license
to practice as a physician with no further right to reinstatement or
reactivation.
In November 2005 the Nursing Assistant Program reinstated the certification of
Jalali Whitten to practice as a nursing assistant (NC10049136).
In November 2005 the Medical Commission placed the license of physician
William S. Yale (MD00014013) on probation for two years. He must comply with
terms and conditions. Yale aided and abetted the unlicensed practice of
medicine.
Mason County
In November 2005 the Nursing Commission charged registered nurse Randall M.
Brower (RN00151044) with unprofessional conduct. The Nevada State Board of
Nursing allegedly took action against his Nevada nursing license.
Pierce County
In November 2005 the Registered Counselor Program charged William T. Haavik,
an applicant to practice as a registered counselor (CA00007355), with
unprofessional conduct. He was convicted of attempting to elude a police vehicle
and criminal mistreatment.
San Juan County
In November 2005 the Dental Commission charged dentist Mark D. Fleming
(DE00007515) with unprofessional conduct. He allegedly provided treatment that
failed to meet the accepted standard.
Snohomish County
In November 2005 the Pharmacy Board charged Shawn Eagleton-Yetter, a
pharmacy technician (VA00044666), with unprofessional conduct. Allegations
against her include diverting controlled substances from her workplace for her
own use.
In November 2005 the Medical Commission withdrew a statement of charges against
physician Wayne E. Garrett (MD00005212).
In November 2005 the Nursing Commission charged registered nurse Lucy M.
Ulbrickson (RN00068421) with unprofessional conduct. Allegations against her
include failing to administer a patient’s medication and failing to adequately
respond to and notify the physician of a change in the patient’s condition.
Spokane County
In November 2005 the Nursing Commission denied the application of Ladonna D.
Black to practice as a licensed practical nurse (CA00017577). She may not
reapply for five years. The Montana Board of Nursing indefinitely suspended
Black’s Montana nursing license.
In November 2005 the Nursing Assistant Program withdrew a statement of charges
against nursing assistant Jackie K. Hughes (NA00090388).
In November 2005 the Registered Counselor Program reinstated the registration of
Elana W. Kemple to practice as a registered counselor (RC00020119). Elana
W. Kemple is also known as Elana W. Marshall and Elana W. Cordoz.
In November 2005 the Nursing Commission charged registered nurse Karen M.
Reichardt (RN00087712) with unprofessional conduct. Allegations against her
include providing care that fell below standard and practicing outside the scope
of a registered nurse.
In November 2005 the Registered Counselor Program charged Roberta J.
Vangrimbergen, an applicant to practice as a registered counselor
(CA00007486), with unprofessional conduct. She was convicted of conspiring to
deliver a controlled substance and possession of a controlled substance. Roberta
J. Vangrimbergen is also known as Roberta J. Deardorff.
Thurston County
In November 2005 the Nursing Commission charged Dara L. Rodriguez, a
licensed practical nurse (LP00050159), with unprofessional conduct. She
allegedly diverted medications and made false entries in records.
Out of State
(Many health care providers hold licenses in more than one state. Department of
Health receives information each month from national groups on disciplined
health care providers. When another state’s regulatory group takes actions, the
equivalent board/commission/program in Washington files unprofessional conduct
charges against the provider’s Washington credential.)
Colorado: In November 2005 the Medical Commission placed the license of
James E. Damon, a physician licensed in Washington (MD00030461), on
probation. He agrees to comply with the terms and conditions of a prior order
entered into with the Colorado Board of Medical Examiners. His practice is
limited to performing independent medical evaluations.
Kansas: In November 2005 the Medical Commission granted the request of
Mark Bryniarski, a physician previously licensed in Washington (ML20006046),
for termination of the terms and conditions of a prior order.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
