State revokes, suspends licenses, certifications, registrations
of health care providers
Department taking more steps to improve patient safety
OLYMPIA • The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department» s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency» s Web site. Click on "Provider Credential Search" on the left hand side of the home page (www.doh.wa.gov). The site includes information about a health care provider» s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In July 2006 the Nursing Assistant Program suspended the registration of nursing assistant Lesia M. Curtis (NA00006715). Her application to practice as a nursing assistant was denied and she may not reapply for two years. Curtis was convicted of animal cruelty and possession of marijuana.
In July 2006 the Department of Health suspended the license of registered nurse Loretta Antoine (RN00138989). She failed to comply with a student loan.
In July 2006 the Medical Commission indefinitely suspended the license of physician Charles R. Flake (MD00020623. He must cease practice and pay a $5,000 fine. Flake had sexual relationships with patients.
In July 2006 the Unlicensed Practice Program issued a Cease and Desist Order against Faith He. She must immediately stop practicing and offering medical procedures without a license. He gave injections to patients, represented herself as a registered nurse and advertised acupuncture and massage services.
In July 2006 the Registered Counselor and Chemical Dependency Programs revoked the credentials of John E. Morris, a registered counselor and chemical dependency professional (RC00014069, CP0001314), for 10 years. Morris was convicted of assault and domestic violence.
In July 2006 the Nursing Commission indefinitely suspended the license of Keli K. Phillips, a registered nurse (RN00136506). She administered medication to a patient without a physician» s order and documented that the medication was ordered by a physician when the order was not authorized.
Pierce County
In June 2006 the Medical Commission indefinitely suspended the license of physician Lance A. May (MD00038397). He treated himself with medications he diverted from his workplace. May was also found guilty of criminal sexual assault on a family member.
Snohomish County
In June 2006 the Pharmacy Board suspended the certification of Jonathan D. Lundberg, a pharmacy technician (VA00047828), for 10 years. On several occasions, Lundberg diverted controlled substances from his workplace.
In July 2006 the Nursing Commission indefinitely suspended the license of Karryn M. Meeker, a registered nurse (RN00086236). She failed to comply with prior limitations against her license.
Spokane County
In July 2006 the Nursing Assistant Program revoked the registration of nursing assistant Corina M. Carpena (NA00060778) for 10 years. She was convicted of theft, forgery and delivering methamphetamine.
Thurston County
In June 2006 the Chiropractic Commission revoked the license of chiropractor Shannon E. Hollister (CH00033850) for 20 years. She was convicted of interstate murder for hire.
Kentucky: In July 2006 the Medical Commission indefinitely suspended the license of David E. Collins, a physician licensed in Washington (MD00041332). In 2005 the Kentucky Board of Medical Licensure restricted his medical license in Kentucky.
Texas: In July 2006 the Medical Commission revoked the license of Jose E. Perezcassar, a physician licensed in Washington (MD00022242), for 10 years. His medical license in Texas was revoked in 2004.
