State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In August 2006 the Massage Program charged Jason A. Cady, a massage practitioner (MA00019947), with unprofessional conduct. Cady allegedly had sexual contact with a client.

Chelan County

In August 2006 the Nursing Assistant Program denied the application of Monica E. Cisneros to practice as a nursing assistant (CA00044264). She may not reapply for three years. Cisneros was convicted of assault.

Clallam County

In August 2006 the Nursing Commission charged registered nurse Janet Anne Cumming (RN00112469) with unprofessional conduct. Cumming allegedly failed to administer or document administration of medications to patients, failed to follow doctors’ orders and was convicted of driving under the influence.

Clark County

In August 2006 the Nursing Assistant Program charged applicant Laverna J. Rhodes (NC10059327) with unprofessional conduct. She had several criminal convictions on record including convictions for burglary and theft.

In August 2006 the Nursing Assistant Program charged applicant Erika L. Waters (CA00045792) with unprofessional conduct. She had been convicted previously of theft and reckless endangerment.

Cowlitz County

In August 2006 the Chemical Dependency Program charged counselor Robert B. Collins (CP00004146) with unprofessional conduct. Collins allegedly misrepresented his business to a probation agency.

In August 2006 the Registered Counselor and Chemical Dependency programs charged counselor Sixto J. Vargas, Jr. (CP00002643, RC00018241) with unprofessional conduct. He had pleaded guilty to public indecency in Oregon.

King County

In August 2006 the Nursing Commission reinstated the license of Loretta Antoine to practice as a registered nurse (RN00138989).

In July 2006 the Psychology Board charged psychologist Stephen J. Cummins (PY00000843) with unprofessional conduct. He was convicted of two counts of domestic violence.

In August 2006 the Nursing Assistant Program charged applicant Lamiiko Q. Franklin (CA00046245) with unprofessional conduct. She had previously been convicted of possession of stolen property.

In August 2006 the Dental Commission entered an Agreed Order with dentist Paul F. Genung (DE0003370). He agrees to comply with terms and conditions, including completing continuing education and paying a $10,000 fine. Genung failed to adequately maintain patient histories containing medication prescribed or dispensed to patients.

In August 2006 the Dental Quality Assurance Commission amended charges against dentist Shohreh Khorsandi (DE00007812). He allegedly performed dental work that was below the standard of care.

In August 2006 the Dental commission charged dentist Hoi W. Lee (DE00004763) with unprofessional conduct. Allegations against Lee include providing services that failed to meet the standard of care.

In August 2006 the Unlicensed Practice Program issued a cease and desist order against Aubrey Hart Sparks, Seattle, for practicing massage without a license. Sparks advertised as a massage therapist on an Internet Web site and in the Seattle Weekly.

In August 2006 the Registered Counselor Program charged Erin M. Stewart, an applicant to practice as a registered counselor (CA00008128), with unprofessional conduct. She was convicted of attempted forgery and obstructing a law officer.

Lewis County

In August 2006 the Nursing Assistant Program denied the application of Jammie L. Rosbach to practice as a nursing assistant (CA00042728). She may not reapply for five years. Rosbach was convicted of theft and taking a motor vehicle without permission.

Mason County

In August 2006 the Chemical Dependency Program amended charges against counselor John C. Gallegos (CP00004090). He allegedly crossed professional boundaries with several clients.

Pierce County

In August 2006 the Massage Program denied the application of Raun J. Sedlock to practice as a massage practitioner (CA00008129). He is unable to practice with reasonable skill and safety.

Spokane County

In August 2006 the Nursing Assistant Program denied the application of Dawn L. Hohn to practice as a nursing assistant (CA00044234). She was convicted of possessing a controlled substance, criminal mistreatment and bail jumping.

In August 2006 the Nursing Assistant Program charged applicant Lori L. Martin (NA00074356) with unprofessional conduct. She had previously been convicted of manufacturing methamphetamine.

In August 2006 the Dental Commission charged dentist Frank Morgan (DE00003843) with unprofessional conduct. He allegedly provided treatment that fell below the standard of care.

Thurston County

In August 2006 the Nursing Commission withdrew a statement of charges against Kenneth Saucke, a licensed practical nurse (LP00027619).

In August 2006 the Nursing Commission charged registered nurse Cheryl H. Westergaard (RN00040824) with unprofessional conduct. Westergaard allegedly failed to comply with previous conditions on her license.

Whatcom County

In August 2006 the Nursing Assistant Program reinstated the certification of Lori Ann Hagins to practice as a nursing assistant (NC10019388).

Yakima County

In August 2006 the Nursing Commission charged registered nurse Sarah E. Glasscock (RN00160178) with unprofessional conduct. Allegations against her include practicing while impaired.

In August 2006 the Nursing Assistant Program charged applicant Terri S. Oakes (CA00046294) with unprofessional conduct. Her license as a licensed practical nurse had previously been suspended.

Out of State

(Many health care providers hold licenses in more than one state. Department of Health receives information each month from national groups on disciplined health care providers. When another state’s regulatory group takes actions, the equivalent board/commission/program in Washington files unprofessional conduct charges against the provider’s Washington credential.)

California: In August 2006 the Medical Commission charged Deborah S. Golob, a physician licensed in Washington (MD00044606), with unprofessional conduct. In 2005 her license to practice medicine in Arizona was restricted.

Idaho: In August 2006 the Veterinary Board reinstated the license of John W. Hoeck to practice as a veterinarian in Washington (VT00007665).

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
