State disciplines health care providers
OLYMPIA • The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department» s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on » Provider Credential Search» on the left hand side of the home page (www.doh.wa.gov). The site includes information about a health care provider» s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Asotin County
In August 2006 the Nursing Commission charged Debbie L. Jones, a licensed practical nurse (LP00029489), with unprofessional conduct. She allegedly presented false documents to her prospective employer.
In August 2006 the Pharmacy Board charged Kenyon N. Lyle, an applicant to practice as a pharmacy intern (CA00056711), with unprofessional conduct. Lyle was convicted of a minor operating a vehicle after alcohol consumption, negligent driving and driving under the influence.
King County
In August 2006 the Chiropractic Commission reinstated the license of Frederick W. Aften to practice as a chiropractor (CH0002303).
In August 2006 the Osteopathic Medicine and Surgery Board charged David D. Asmussen, an osteopathic physician (OP00000658), with unprofessional conduct. He allegedly provided care that fell below the standard.
In August 2006 the Mental Health Counselor Program placed the license of Roberta A. Banks, a mental health counselor (LH00004812), on probation for two years. She must comply with terms and conditions. Banks violated rules regulating the practice of mental health counselors.
In September 2006 the Medical Commission charged physician Thomas A. Clarkson (MD00036286) with unprofessional conduct. Allegations against him include using information in patients» medical charts for an illegitimate purpose.
In August 2006 the Respiratory Care Program denied the application of Jeffrey T. Hutchens to practice as a respiratory care practitioner (CA00001928). He was convicted of driving under the influence.
In August 2006 the Medical Commission entered an Agreed Order with physician Timothy H. Massey (MD00030689). He is reprimanded for failing to cooperate with an investigation.
In September 2006 the Medical Commission amended a statement of charges against physician assistant Paul J. Nelson (PA10003198). He is being charged with unprofessional conduct for allegations including examining a patient in an inappropriate manner and failing to cooperate with the commission.
In August 2006 the Pharmacy Board charged Michelle M. Roberts, a pharmacy assistant (VB00060368), with unprofessional conduct. She allegedly diverted controlled substances from her place of employment and from co-workers.
In August 2006 the Health Care Assistant Program amended a statement of charges against Ahmad A. As Sadiq, a health care assistant (HC00143757). He is charged with unprofessional conduct for being convicted of possessing a controlled substance, possessing stolen property and obstructing a public servant. Ahmad A. Sadiq is also known Ahmad A. Assadiq, Dyles Rico Cannon, Rico Dyles Cannon and Rico Cannon.
In August 2006 the Pharmacy Board charged Alecia A. Sutter, a pharmacy assistant (VB00058784), with unprofessional conduct. Allegations against her include diverting various controlled substances from her workplace for her own use.
Kitsap County
In August 2006 the Nursing Assistant Program denied the application of Malissa H. Fyffe to practice as a nursing assistant (CA00044083). She may not reapply for five years. Fyffe was convicted of assault.
In August 2006 the Nursing Assistant Program charged Danielle T. Nguyen, an applicant to practice as a nursing assistant (CA00046368), with unprofessional conduct. His pharmacy technician credential was placed on probation for two years.
Lewis County
In August 2006 the Nursing Assistant Program denied the application of Deanna R. Kleemeyer to practice as a nursing assistant (CA00045490). She may not reapply for five years. Kleemeyer was convicted of identity theft.
Mason County
In August 2006 the Nursing Commission reinstated the license of Shelley A. Dixon to practice as a licensed practical nurse (LP00018169). Her license is placed on probation and she must comply with terms and conditions. Shelley A. Dixon is also known as Shelley A. Dodge.
In August 2006 the Nursing Assistant Program denied the application of Edwin M. Groce to practice as a nursing assistant (CA00045342). He may not reapply for 10 years. He was convicted of hit and run and possession of a dangerous weapon.
In September 2006 the Nursing Assistant Program charged nursing assistant Christine L. Windley (NC10079636) with unprofessional conduct. The Department of Social and Health Services restricted Windley from employment at a nursing home or in a position involving caring for vulnerable adults.
Okanogan County
In August 2006 the Chemical Dependency Professional and Registered Counselor Programs charged Gary Sims, a chemical dependency professional and registered counselor (CP00001819, RC00026808), with unprofessional conduct. Allegations against him include forging patient signatures and lying about doing testing on patients.
Pierce County
In August 2006 the Registered Counselor Program reinstated the registration of Garuba D. Akinniyi to practice as a registered counselor (RC00046494).
In August 2006 the Medical Commission withdrew a statement of charges against physician James H. Greene (MD00028865).
In August 2006 the Nursing Commission amended a statement of charges against registered nurse Luis Ibarra (RN00144561). He is being charged with unprofessional conduct for allegedly making sexually suggestive comments to a patient.
In August 2006 the Registered Counselor program denied the application of Kimberly D. Mays to practice as a registered counselor (CA00007786). She was convicted of several counts of unlawful possession of a controlled substance.
In August 2006 the Registered Counselor Program granted the application of Leah S. Niccolocci to practice as a registered counselor (CA00008035). Her registration is placed on probation for two years and she must comply with terms and conditions. Niccolocci was convicted of unlawful possession of a controlled substance.
In August 2006 the Massage Program charged Ann Marie Taylor, a massage practitioner (MA00004584), with unprofessional conduct. Allegations against her include allowing an unlicensed individual to practice massage and practicing herself without a valid license.
Spokane County
In August 2006 the Nursing Assistant Program charged Carrie L. Wesolek, an applicant to practice as a nursing assistant (CA000045053), with unprofessional conduct. She was convicted of malicious mischief.
Thurston County
In august 2006 the Nursing Commission entered an Agreed Order with registered nurse Dana L. Noble (RN00155430). She agrees to comply with terms and conditions, including paying a fine and submitting reports. Noble failed to communicate a change in a patient» s condition in a timely manner and failed to take immediate nursing intervention.
Yakima County
In August 2006 the Nursing Commission entered into an Agreed Order with registered nurse Sarah E. Glasscock (RN00160178). She agrees to comply with terms and conditions. Glasscock was incapacitated while at work.
Out of State
Idaho: In August 2006 the Nursing Commission granted the application of Lorri L. Champion to practice as a registered nurse in Washington (CA00063087). Her license is placed on probation for two years and she must comply with terms and conditions. Champion was convicted of forgery and possessing a controlled substance in the state of Idaho.
Michigan: In August 2006 the Medical Commission charged Mark A. Mattos, a physician licensed in Washington (MD00040481), with unprofessional conduct. Allegations against him include failing to comply with the terms and conditions of a prior order.
Texas: In August 2006 the Nursing Commission charged Reue L. Verbus, a registered nurse licensed in Washington (RN00159700), with unprofessional conduct. Allegations against him include fondling a patient» s breasts and making romantic advances towards the patient.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
