OLYMPIA - The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency’s Web site. Click on “Provider Credential Search” on the left hand side of the home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Benton County
In September 2006 the Medical Commission indefinitely suspended the license of physician Janet S. Arnold (MD00025273). She failed to comply with terms and conditions against her license.
Clark County
In August 2006 the Health Care Assistants Program revoked the certification of Mary M. Wakeham, a health care assistant (HC000143071), for three years. She had a friend forge the signature of a physician on a prescription.
Cowlitz County
In August 2006 the X-Ray Technician Program indefinitely suspended the registration of x-ray technician Lori A. Johnson (XT00005056). She used a patient’s prescription for a controlled substance.
Grant County
In September 2006 the Department of Health suspended the license of nursing assistant Ricky L. Blocker (NC10063891). He defaulted on a student loan.
In August 2006 the Registered Counselor Program revoked the registration of registered counselor Kathy Gardner (RC00048580) until 2010. She was convicted of theft and forgery.
King County
In September 2006 the Chiropractic Commission entered an Agreed Order with chiropractor Patric R. Golob (CH00001747). He agrees to surrender his license and not practice as a chiropractor.
In September 2006 the Pharmacy Board revoked the license of David J. Grech, a pharmacy technician (VA00043519), for 10 years. He diverted a controlled substance from his workplace for his own use.
In August 2006 the Nursing Commission indefinitely suspended the license of Jessie A. Hollins, a licensed practical nurse (LP00018654). She failed to comply with prior terms and conditions placed against her license.
In August 2006 the Nursing Commission indefinitely suspended the license of registered nurse Laurie R. Kiss (RN00115186). She failed to comply with the terms and conditions of a prior order.
In September 2006 the Health Care Assistant Program revoked the certification of health care assistant Colleen E. Kring (HC00136392) for three years. Kring was fired from her employment for falsifying prescriptions and taking equipment and medications from her workplace to sell on the streets. She was also convicted of soliciting possession of a controlled substance.
In September 2006 the Nursing Commission indefinitely suspended the license of registered nurse Barbara L. Marshall (RN00104189). She failed to comply with prior terms and conditions on her license.
In August 2006 the Nursing Commission indefinitely suspended the license of Paramjyoti Paramjyoti, a licensed practical nurse (LP00036200). She failed to comply with terms and conditions placed against her license.
In September 2006 the Nursing Commission suspended the license of registered nurse Tamara M. Perry (RN00114237). She failed to comply with prior terms and conditions against her license.
In September 2006 the Unlicensed Practice Program issued a Cease and Desist Order against Bobbi Jo Sanderson. She must immediately stop the unlicensed practice of medicine, nursing and health care assisting.
Kitsap County
In September 2006 the Nursing Commission indefinitely suspended Stephanie L. Henzel, a licensed practical nurse (LP00036991). She failed to comply with the terms and conditions of a prior order.
Mason County
In September 2006 the Nursing Commission indefinitely suspended the license of Thomas C. Johnson, a licensed practical nurse (LP00050197). He failed to comply terms and conditions placed on his license.
Pierce County
In September 2006 the Nursing Assistant Program revoked the certification of nursing assistant Elizabeth A. Johnson (NC10075808) for five years. She gave patients medication they were not prescribed and gave a patient medication at the wrong time.
In August 2006 the Unlicensed Practice Program issued a Cease and Desist Order against Lisa Ogdon. She must immediately stop practicing as a dental assistant. She does not have a license to practice dentistry or dental hygiene.
In August 2006 the Unlicensed Practice Program issued a Cease and Desist Order against Tobert Timmer. He must immediately stop practicing as a counselor and chemical dependency professional. Timmer does not have any credential to practice as a health care provider.
Snohomish County
In September 2006 the Chiropractic Commission indefinitely suspended the license of chiropractor Keith D. Walker (CH00034061). He failed to comply with the terms of probation placed against his license by seeing female patients without a female monitor present. Walker also continued to treat patients after his license expired.
Whatcom County
In September 2006 the Nursing Commission indefinitely suspended the license of registered nurse Jeanine M. Walters (RN00157356). In May 2005 she surrendered her license to practice as a registered nurse in California.
Arizona: In September 2006 the Respiratory Care Practitioner Program revoked the license of Willem G. Cudney, a respiratory care practitioner licensed in Washington (LR00003072). He was convicted of soliciting to commit possession of a controlled substance. He also sent a forged letter in an effort to get his license reinstated.
Arizona: In September 2006 the Nursing Commission indefinitely suspended the license of Shubulola T. Johnson, a licensed practical nurse licensed in Washington (LP00050112). She failed to label a medication intended for a patient. As a result, the medication was given to the wrong patient.
California: In September 2006 the Nursing Commission revoked the license of Michael J. Bradford, a registered nurse licensed in Washington (RN00157161) for 10 years. He diverted medicine from a patient’s IV bag for his own use. His Arizona license was revoked in May 2006.
California: In September 2006 the Nursing Commission indefinitely suspended Criss J. Helms, a registered nurse licensed in Washington (RN00159395). In May 2006 he surrendered his license to practice as a registered nurse in Arizona.
Florida: In September 2006 the Nursing Commission indefinitely suspended the license of Lisa A. Winegardner, a registered nurse licensed in Washington (RN00147411). She tested positive for cocaine.
Idaho: In August 2006 the Nursing Commission suspended the license of Michael J. Sahlie, a registered nurse licensed in Washington (RN00124994), for one year. Sahlie failed to comply with prior limitations placed against his license.
Oregon: In August 2006 the Nursing Commission indefinitely suspended the license of Sabrina Bird, a licensed practical nurse licensed in Washington (LP00051036). She was convicted of obtaining a controlled substance by fraud and uttering a false or forged prescription.
Michigan: In August 2006 the Unlicensed Practice Program issued a Cease and Desist Order against Robin Vadala. She must immediately stop the unlicensed practice of nursing. While working in Washington, Vadala represented herself as a licensed practical nurse; she does not have a credential to practice as a health care provider in Washington.
