State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In September 2006 the Dental Commission withdrew a statement of charges against dentist Thao Chu (DE00009778).

In September 2006 the Physical Therapy Board charged Danielle R. Sutter, an applicant to practice as a physical therapist (CA00004810), with unprofessional conduct. She was prohibited from applying for a license with the Physical Therapist Board in Oregon.

Jefferson County

In September 2006 the Pharmacy Board placed the license of Jina M. Johnson, a pharmacy technician (VA00055740), on probation for two years. She must comply with terms and conditions. Johnson took prescriptions from her workplace without paying for them and pled guilty to theft.

King County

In September 2006 the Medical Commission charged physician John J. Fina (MD00033991) with unprofessional conduct. Allegations against him include prescribing large amounts of injectable drugs without follow-up care, failing to provide for informed consent, and failing to refund a patient’s money when the patient refused further treatment.

In September 2006 the Dental Commission withdrew a statement of charges against dentist Duane L. Jones (DE00006522).

In September 2006 the Acupuncture Program placed the license of acupuncturist Jianjun Jian (AC00000293) on probation for one year. He must comply with terms and conditions. Jian billed for treatment not provided, failed to provide a patient’s records and represented himself as a doctor.

In September 2006 the Pharmacy Board reinstated the license of Roshan U. Siddiqui to practice as a pharmacist (PH00011835).

Kitsap County

In September 2006 the Dental Commission entered an Agreed Order with dentist Hamidreza Barakhshan (DE00009257). He agrees to comply with terms and conditions, including paying a $5,000 fine. He failed to adequately record or maintain treatment records.

In September 2006 the Nursing Commission charged registered nurse Karl Sonnenberg (RN00133794) with unprofessional conduct. He allegedly failed to comply with the terms and conditions of a prior order.

Pierce County

In September 2006 the Optometry Board charged optometrist William S. Church (OD00000869) with unprofessional conduct. He allegedly wrote a prescription for medication which he does not have authorization to prescribe as an optometrist.

In September 2006 the Nursing Assistant Program charged Hadassah M. Cohen, an applicant to practice as a nursing assistant (CA00046394), with unprofessional conduct. Cohen’s license to practice as a licensed practical nurse is subject to probation.

In September 2006 the Nursing Assistant Program charged Larry H. Hendricks, an applicant to practice as a nursing assistant (CA00046295), with unprofessional conduct. He had restrictions placed against his credential to practice as a registered counselor.

In August 2006 the Marriage and Family Therapist and Mental Health Counselor Programs charged Ann MacGowan, a marriage and family therapist and mental health counselor (LF00002080, LH00006172), with unprofessional conduct. She allegedly engaged in a social and/or romantic relationship with a client.

In September 2006 the Dental Commission withdrew a statement of charges against dentist Don S. Moore (DE00008678).

In August 2006 the Medical Commission charged physician Manuel P. Posadas (MD00021612) with unprofessional conduct. Allegations against him include failing to safeguard medical records.

Skamania County

In August 2006 the Chemical Dependency Professional and Registered Counselor Programs charged Kristal L. Rosebrook, a chemical dependency professional and registered counselor (CP00003106, RC00037528), with unprofessional conduct. She had sexual contact with a patient.

Snohomish County

In September 2006 the Nursing Assistant Program charged nursing assistant Anthony A. Manzano (NC10057020) with unprofessional conduct. He was convicted of assault.

Spokane County

In September 2006 the Nursing Assistant Program denied the application of Regnell C.W. Fuller to reactivate his certification to practice as a nursing assistant. He was convicted of unlawful issuance of bank checks.

In August 2006 the Dental Commission withdrew a statement of charges against dentist Don G. Marshall (DE00007116).

In September 2006 the Pharmacy Board granted the request of Kevin Pittsley for reinstatement of his certification to practice as a pharmacy technician (VA00045728).

In September 2006 the Nursing Commission charged registered nurse Colleen J. Wagner (RN00046949) with unprofessional conduct. She allegedly failed to comply with the terms and conditions of a prior order.

In September 2006 the Nursing Assistant Program reinstated the certification of Stephanie E. Watson to practice as a nursing assistant (NA00177559).

Stevens County

In September 2006 the Unlicensed Practice Program issued a Notice of Intent to issue a Cease and Desist Order against Rob McClintock. He allegedly represented himself as a doctor of naturopathy. He does not have a credential to practice health care.

Thurston County

In September 2006 the Nursing Commission placed the license of registered nurse Dean P. DeMulling (RN00085868) on a stayed suspension for five years. He must comply with terms and conditions. DeMulling inappropriately touched female patients.

In September 2006 the Nursing Assistant Program amended a statement of charges against nursing assistant Charles K. Kimball (NA00179462). He is charged with unprofessional conduct for allegedly hitting patients. Kimball has been prohibited from working in nursing homes.

In September 2006 the Nursing Assistant Program charged nursing assistant Traci J. McAllister (NC10089334) with unprofessional conduct. She allegedly stole a watch belonging to a patient and was convicted of theft.

In September 2006 the Nursing Assistant Program reinstated the certification of Alberta Lee Misner to practice as a nursing assistant (NC10019167). Alberta Lee Misner is also known as Alberta L. Guerin.

Whatcom County

In September 2006 the Chiropractic Commission charged Lance M. Mahoney, an applicant to practice as a chiropractor (CA00002814), with unprofessional conduct. His license to practice as a chiropractor in Iowa was indefinitely suspended.

Out of State

Idaho: In August 2006 the Medical Commission denied the application of Cynthia L. Clinkingbeard to reactivate her license to practice as a physician in Washington (MD00019381). She is unable to practice with reasonable skill and safety.

Oregon: In September 2006 the Dental Commission charged Jamshid Faghih, a dentist licensed in Washington (DE00008602), with unprofessional conduct. He allegedly provided treatment that failed to meet the standard of care.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
