State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency’s Web site. Click on "Provider Credential Search" on the left hand side of the home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Cowlitz County
In October 2006 the Chemical Dependency Professional Program indefinitely suspended the certification of Robert B. Collins, a chemical dependency professional (CP00004146). He conducted alcohol/drug assessments on clients when his business was not a state certified drug and alcohol agency. He submitted documents to courts and to the licensing agency using letterhead and records from another state certified drug and alcohol facility.
King County
In October 2006 the Registered Counselor Program suspended registered counselor Devlin J. Crose (RC000033286) for five years. Crose had a personal relationship with a client.
In September 2006 the Nursing Assistant Program revoked the credential of nursing assistant Calvin Lewis (NC10085843) for 10 years. He had sex with a patient.
In October 2006 the Registered Counselor Program revoked the registration of Amy L. Russell, a registered counselor (RC00052803), for three years. She was convicted of identity theft and forgery.
In October 2006 the Health Care Assistant Program indefinitely suspended the certification of nursing assistant Ahmad A. As Sadiq (HC00143757). He concealed several criminal convictions, including possessing stolen property and a controlled substance. Ahmad A. Sadiq is also known as Ahmad A. Assadiq, Dyles Rico Cannon, Rico Dyles Cannon and Rico Cannon.
In October 2006 the Dental Commission revoked the license of dentist Douglas A. Seymour (DE00006613) for seven years. He billed patients for services not provided.
In October 2006 the Nursing Commission revoked the credentials of Diane E. Young-Ackley, a registered nurse and licensed practical nurse (RN00096422, LP00034930). In May 2006, her credential to practice nursing in Arizona was revoked.
Lewis County
In October 2006 the Nursing Commission revoked the license of Gilbert L. Taddei, a licensed practical nurse (LP00052557), for 20 years. He was convicted of unlawful possession of a firearm and ammunition, felon in possession of a firearm and identity theft. Gilbert L. Taddei is also known as Charles Marion White.
Mason County
In October 2006 the Nursing Commission revoked the license of Nancy L. Barnett, a licensed practical nurse (LP00020548), for 10 years. She was convicted of assault, forgery, violating a no contact order, displaying a deadly weapon and driving under the influence.
In October 2006 the Chemical Dependency Professional Program revoked the certification of John C. Gallegos, a chemical dependency professional (CP00004090), for 20 years. He crossed professional boundaries with clients, including making inappropriate comments, exposing himself and hugging and kissing them.
In October 2006 the Nursing Assistant Program revoked the certification of Christine L. Windley, a nursing assistant (NC10079636), for 10 years. In July 2006 the Department of Social and Health Services restricted Windley from working in nursing homes or caring for vulnerable adults.
Pierce County
In August 2006 the Board of Hearing and Speech suspended the license of Larry L. Balles, a hearing instrument fitter/dispenser (HA00003842), for three years. He admitted to smoking marijuana.
Skagit County
In September 2006 the Chiropractic Commission revoked the license of chiropractor Daniel K. York (CH00000868) for 10 years. He was convicted of raping a child and child molestation. His license was immediately suspended in August 2006.
Snohomish County
In September 2006 the Nursing Assistant Program revoked the certification of nursing assistant Lamin Darboe (NC10029559) for 20 years. He sexually assaulted a patient. His certification was immediately suspended in August 2006.
Thurston County
In October 2006 the Nursing Commission revoked the license of registered nurse Gregory Hallen (RN00064556). In February 2006 the California Board of Registered Nursing revoked his license for unprofessional conduct. This included charting and medication errors, possession of controlled substances and failing to maintain infection control.
In October 2006 the Nursing Assistant Program revoked the certification of nursing assistant Traci J. McAllister (NC10089334) for five years. She stole a watch belonging to a patient and was convicted of theft.
