State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency’s Web site. Click on "Provider Credential Search" on the left hand side of the home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Immediate suspensions

(A new law that went into effect in June 2006 requires the department to immediately suspend health care providers who have been banned from practicing in other states.)

In October 2006 the Medical Commission summarily suspended Peter N. Brawn (MD00014691) from practicing medicine in Washington. In 2005 his credential to practice medicine in Florida was suspended for two years.

In October 2006 the Nursing Commission summarily suspended Theresa G. Satiacum (RN00124014) from practicing nursing in Washington. In 2004 her Montana registered nursing license was indefinitely suspended.

Okanogan County

In October 2006 the Nursing Assistant Program revoked the certification of nursing assistant Yolanda E. Valdavinos (NC10071581) for 10 years. She was convicted of manslaughter. Yolanda E. Valdavinos is also known Yolanda E. DeVon.

Pierce County

In October 2006 the Nursing Commission indefinitely suspended the license of registered nurse Carlotta Getzin Martinson (RN00103069). She failed to comply with terms and conditions against her license.

Snohomish County

In October 2006 the Nursing Assistant Program revoked the certification of Jerry L. Murry, a nursing assistant (NC10067120), for 10 years. He coerced and/or forced a 16-year-old girl to have sex with him and asked her to work as a prostitute for him.

Thurston County

In October 2006 the Licensed Mental Health Counselor Program revoked the license of mental health counselor Barrette D. Green (LH00003833) for 10 years. Green made inappropriate sexual comments to women co-workers, touched them inappropriately and threatened them.

Whatcom County

In October 2006 the Chemical Dependency Professional, Licensed Mental Health Counselor and Marriage and Family Therapy Counselor Programs entered an Agreed Order with Duane W. Stewart (CP00002363, LH00004369, LF00001124). He agrees to surrender all three of his health care credentials. He crossed professional boundaries with a patient.

Out of State

Nevada: In October 2006 the Dental Commission suspended the license of Anthony J. Lamancusa, a dentist licensed in Washington (DE00007478). His license to practice as a dentist in Nevada had restrictions.

Oregon: In October 2006 the Chemical Dependency Professional and the Registered Counselor Programs suspended the credentials of George J. Beal, a chemical dependency professional and registered counselor in Washinton (CP00005202, RC00044618), for two years. After two years, Beal’s credentials will be placed on a stayed suspension and he must comply with terms and conditions. Beal signed a supervisor’s and an administrator’s name without approval.
