State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on "Provider Credential Search" on the left hand side of the home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Asotin County

In October 2006 the Nursing Commission charged registered nurse Sam Olson (RN00056815) with unprofessional conduct. He allegedly repeatedly physically and verbally abused a patient.

Chelan County

In October 2006 the Massage Program placed the license of massage practitioner Jason A. Cady (MA00019947) on probation for five years. He must comply with terms and conditions. Cady had sexual contact with a client.

Clark County

In October 2006 the Nursing Commission placed the license of Hope D. Bonell, a licensed practical nurse (LP00012795), on probation for one year. She must comply with terms and conditions. Bonell administered the wrong medication and the wrong dosage of medication to patients.

In October 2006 the Physical Therapy Board charged physical therapist Michael McQuivey (PT00005278) with unprofessional conduct. Allegations against him include allowing a patient to be seen by an aide without his required supervision.

Cowlitz County

In October 2006 the Registered Counselor Program placed registered counselor Eli A. Bouchea (RC00020663) on a stayed suspension for two years. He must comply with terms and conditions. Bouchea misrepresented his academic credentials and/or education.

Franklin County

In October 2006 the Registered Counselor Program charged registered counselor Servando Jasso (RC00014566) with unprofessional conduct. He allegedly physically and sexually abused children.

Grant County

In October 2006 the Pharmacy Board charged Joben U. Nuesse, an applicant to practice as a pharmacy assistant (CA00062093), with unprofessional conduct. He is allegedly unable to practice with reasonable skill and safety.

King County

In October 2006 the Nursing Assistant Program charged Davina L. Dunmore, an applicant to practice as a nursing assistant (CA00045719), with unprofessional conduct. She was convicted of taking a motor vehicle without permission and attempting to elude.

In October 2006 the Nursing Commission charged registered nurse Patricia A. Foley (RN00130591) with unprofessional conduct. Allegations against her include diverting narcotics from her workplace, falling asleep during her shift and making inaccurate documentation regarding narcotic administration for a patient.

In October 2006 the Nursing Assistant Program charged nursing assistant Gordon A. Roe (NC10088949) with unprofessional conduct. Allegations against him include failing to assist patients and bruising a patient.

In October 2006 the Physical Therapy Board charged Kris D. Schmidt, a physical therapist (PT00007210), with unprofessional conduct. She allegedly practiced while her license was expired.

In October 2006 the Nursing Commission entered an Agreed Order with Tsitsi E. Smith, a licensed practical order (LP00030190). She agrees to comply with terms and conditions, including taking classes and paying a fine. Smith failed to adequately assess and document a change in a patient’s condition.

Pierce County

In October 2006 the Nursing Assistant Program charged nursing assistant Charles A. Bryant (NC10024017) with unprofessional conduct. He tested positive for cocaine.

In October 2006 the Registered Counselor Program charged Michael L. Nye, an applicant to practice as a registered counselor (CA00008455), with unprofessional conduct. He was convicted of possessing a controlled substance.

In October 2006 the Nursing Commission charged registered nurse Patrick M. Shaffer (RN00148173) with unprofessional conduct. He allegedly took a controlled substance from a dead patient and was convicted of grave stealing.

San Juan County

In October 2006 the Pharmacy Board charged pharmacist Claudia D. Bradley (PH00021524) with unprofessional conduct. She allegedly failed to comply with terms and conditions against her license.

In October 2006 the Dental Commission entered an Agreed Order with dentist Mark D. Fleming (DE00007515). He agrees to comply with terms and conditions, including paying a $10,000 and completing coursework. Fleming provided care to a patient that fell below the standard.

Skagit County

In October 2006 the Nursing Assistant Program charged Jacqueline A. Norman, an applicant to practice as a nursing assistant (CA00039009), with unprofessional conduct. She was convicted of assault.

Snohomish County

In October 2006 the Nursing Commission placed the license of Iris E. Rottner, a licensed practical nurse (LP00053164), on a stayed suspension for 18 months. She must comply with terms and conditions. Rottner fell asleep while at work.

Spokane County

In October 2006 the Nursing Commission charged Diane S. Cox, a registered nurse (RN00095701), with unprofessional conduct. She allegedly had disciplinary action taken against her Oregon nursing license. Diane S. Cox is also known as Diane S. Rude.

In October 2006 the Nursing Commission amended a statement of charges against Bonnie L. Schuler, a licensed practical nurse (LP00050139). She is charged with unprofessional conduct for a criminal conviction and having her Idaho nursing license indefinitely suspended.

Thurston County

In October 2006 the Nursing Commission charged Elizabeth M. Anderson, an advanced registered nurse practitioner (AP30005512), with unprofessional conduct. Allegations against her include inappropriately prescribing narcotics to other nurses.

In October 2006 the Registered Counselor Program dismissed charges against Wayne E. Kritsberg, a registered counselor (RC00015918).

In October 2006 the Registered Counselor Program denied the application of Jack L. Roberts to practice as a registered counselor (CA00007957).He may not reapply until August 2008. Roberts was convicted of taking a motor vehicle without permission, forgery and residential burglary.

In October 2006 the Nursing Assistant Program charged nursing assistant Teresa M. Wood (NC10018346) with unprofessional conduct. She allegedly failed to comply with prior terms and conditions against her credential.

Whatcom County

In October 2006 the Medical Commission amended a statement of charges against physician Robert S. Kahn (MD00038546). He is being charged with unprofessional conduct for allegations including diverting controlled substances from his workplace and having a sexual relationship with a patient.

In October 2006 the Unlicensed Practice Program issued a Notice of Intent to issue a Cease and Desist Order against Liquin Lin. She allegedly provided massages for a fee to customers at the Bellis Fair Mall in Bellingham. Lin does not have a license to practice massage.

In October 2006 the Unlicensed Practice Program issued a Notice of Intent to issue a Cease and Desist Order against Jiachang Pu. She allegedly provided massages for a fee to customers at the Bellis Fair Mall in Bellingham. Pu does not have a license to practice massage.

Out of State

California: In October 2006 the Podiatric Medical Board charged Brandon J. Hawkins, an applicant to practice as a podiatric physician in Washington (CA00000300), with unprofessional conduct. While a podiatry resident in California, Hawkins lied about being credentialed to use a machine and lied about how long he had attended the residency program.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
