State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency’s Web site. Click on "Provider Credential Search" on the left hand side of the home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Immediate suspensions

(A new law that went into effect in June 2006 requires the department to immediately suspend health care providers who have been banned from practicing in other states.)

In December 2006 the Medical commission summarily suspended the license of physician John R. Stoianoff. His Idaho license was revoked for failure to comply with an order issued by the Idaho Board of Medicine.

Franklin County

In December 2006 the Chemical Dependency Professional Program revoked the license of Wayne A. Jenkins, a chemical dependency professional (CP00000981), for five years. Jenkins had inappropriate relationships with clients, including having sex with them and kissing them.

King County

In December 2006 the Radiologic Technologist Program revoked the credential of radiologic technologist Sasha R. Bryant (RT00004525) for 10 years. Bryant’s credential was immediately suspended in November 2006 due to a felony conviction for second degree burglary with sexual motivation. Sasha R. Bryant is also known as Rodney Terderial Bryant.

In November 2006 the Unlicensed Practice Program issued a Cease and Desist Order against Mona Burgess. She must immediately stop the unlicensed practice of a nursing assistant. Burgess must also pay a $10,000 fine. While working at Sipes Adult Homecare in SeaTac, she represented herself as holding a nursing assistant certification.

In December 2006 the Nursing Commission indefinitely suspended the license of Mimi O. Etherington, a licensed practical nurse (LP00048872). She failed to comply with prior terms and conditions against her license.

In December 2006 the Nursing Commission indefinitely suspended the license of registered nurse Patricia A. Foley (RN00130591). She diverted narcotics from her workplace, fell asleep during her shift and made inaccurate documentation.

In December 2006 the Dental Commission revoked the license of dentist Hoi W. Lee (DE00004763) for 10 years. Lee performed services that fell below standard and failed to keep appropriate charts for patients.

In November 2006 the Secretary of Health indefinitely suspended the credentials of Jeffrey C. Padgett. He has credentials to practice as a registered counselor (RC00033640), a chemical dependency professional (CP00003648) and a massage practitioner (MA00017172). He crossed professional boundaries with a client.

In December 2006 the Nursing Commission suspended the license of Cynthia Peterson, a licensed practical nurse (LP00019197), for three years. She must comply with terms and conditions. Peterson failed to comply with a prior order.

In December 2006 the Nursing Commission suspended the license of Daniel A. Spiewak, a licensed practical nurse (LP00030392), for 42 months. Spiewak failed to comply with the terms and conditions of a prior order.

Mason County

In December 2006 the Nursing Assistant Program revoked the credential of nursing assistant Dannyell R. Miller (NA00071876) for 10 years. She stole checks and money from a patient. Dannyell R. Miller is also known as Dannyell R. Derrick.

Pierce County

In December 2006 the Nursing Assistant Program indefinitely suspended the credential of Charles A. Bryant, a nursing assistant (NC10024017). He tested positive for cocaine in a drug screening.

In December 2006 the Unlicensed Practice Program issued a Cease and Desist Order against Myong Chow. He must immediately stop providing acupuncture and/or massage. Chow does not have a credential to practice as a health care provider.

In November 2006 the Osteopathic Board indefinitely suspended the license of Kent R. Gebhardt, an osteopathic physician (OP00001235). He viewed pornography at work, removed files from his workplace without permission and abandoned patients. The Pierce County Prosecuting Attorney accused Gebhardt of possessing cocaine.

In December 2006 the Nursing Commission suspended the license of registered nurse Linell M. Jones (RN00056013) for five years. Jones failed to comply with prior terms and conditions placed against her license.

In December 2006 the Nursing Commission indefinitely suspended the license of Susan H. McGrady, a licensed practical nurse (LP00023883). She failed to comply with prior terms and conditions against her license. Susan H. McGrady is also known as Susan Duffield.

In November 2006 the Medical Commission entered an Agreed Order with Susan S. Predmore. She surrendered her license to practice as a physician (MD00026814).

Snohomish County

In December 2006 the Unlicensed Practice Program issued a Cease and Desist Order against Richard A. Lester. He must immediately stop practicing as an unlicensed chiropractor.

In December 2006 the Nursing Commission suspended the license of Max Duane Whipple (RN00124872) for 57 months. He diverted controlled substances from his workplace and failed to comply with prior terms and conditions against his license.

Yakima County

In December 2006 the Licensed Mental Health Counselor Program indefinitely suspended the license of Craig L. Gilley, a mental health counselor (LH00003826). He performed a form of acupressure on a client and touched the client under the client’s clothing.
