State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on "Provider Credential Search" on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In September 2007 the Nursing Assistant Program denied the application of nursing assistant applicant Maryann V. Decknadel (CA00048243). She was convicted of theft on two different occasions. She cannot reapply for five years.

In September 2007 the Registered Counselor Program charged registered counselor applicant Tana M. Freeman (RC00057842) with unprofessional conduct. She was convicted of assault – domestic violence on two different occasions and violation of a domestic violence order.

In September 2007 the Nursing Assistant Program reinstated the registration of registered nursing assistant Cheryl L. Schneider (NA00153325). She is also known as Cheryl L. Storme.

King County

In September 2007 the Occupational Therapy Practice Board charged occupational therapy assistant Warren McPherson (OC00000182) with unprofessional conduct. McPherson’s license to practice as a registered nurse is currently suspended.

In September 2007 the Nursing Assistant Program charged certified nursing assistant Masoud A. Mohamed (NC10074829) with unprofessional conduct. He was convicted of violation of a no contact order and domestic violence. Mohamed allegedly failed to respond to department inquiries regarding the convictions.

In September 2007 the Chiropractic Commission charged chiropractor Lawrence P. Stanton (CH00002345) with unprofessional conduct. He allegedly billed for treatments not documented, failed to provide medical records to the patient’s insurance company and failed to comply with previous terms and conditions set against his license.

In September 2007 the Chiropractic Commission amended charges against chiropractor Mark J. Svetcos (CH00034350) for unprofessional conduct. He allegedly had sexual relationships with some of his patients. His license is suspended pending further proceedings.

Kitsap County

In September 2007 the Registered Counselor Program charged registered counselor applicant Richard S. Lott (RC00058416) with unprofessional conduct. He was convicted of possession of methamphetamine and taking a motor vehicle without permission.

Kittitas County

In September 2007 the Chemical Dependency Professional Program charged chemical dependency professional Toni Johnson (CP00003443) with unprofessional conduct. She allegedly failed to keep adequate client records.

Lewis County

In September 2007 the X-Ray Technician Program charged x-ray technician Jennifer L. Rone (XT00004362) with unprofessional conduct. She allegedly took drugs from her place of employment for her own use and forged prescriptions for legend drugs. Her license to practice as a health care assistant was suspended.

Pierce County

In September 2007 the Chiropractic Commission charged chiropractor Todd B. Atkins (CH00002354) with unprofessional conduct. He allegedly failed to comply with previous terms and conditions set against his license.

In September 2007 the Health Care Assistant Program charged health care assistant Barbara J. Brewer (HC00141309) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions placed against her license.

In September 2007 the Office of Emergency Medical Services and Trauma System charged paramedic Michael J. Drashil (0117760) with unprofessional conduct. He allegedly practiced below the standard of care when a patient’s gurney tipped over resulting in injury.

In September 2007 the Occupational Therapist Practice Board denied the reactivation application of occupational therapist Michael Holst (OT00001885). He practiced with an expired license and altered his license. He cannot reapply until he has met the conditions set in the agreed order.

In September 2007 the Chemical Dependency Professional Program charged chemical dependency professional applicant O. C. K. Leontine (CP00006093) with unprofessional conduct. His registered counselor credential is suspended.

In September 2007 the Registered Counselor Program charged registered counselor applicant Kyle W. Warner (RC00058011) with unprofessional conduct. He was convicted of possession of a stolen firearm.

Snohomish County

In September 2007 the Chiropractic Commission charged chiropractor Marleen L. Meinzer (CH00003432) with unprofessional conduct. Meinzer allegedly failed to adequately document treatment of a patient. She is also known as Marleen L. Brozovich.

In September 2007 the Nursing Commission reinstated the license of licensed practical nurse Sharon J. Miller (LP00012453).

In September 2007 the Health Care Assistant Program charged health care assistant Jan M. Myers (HC00147527) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

In September 2007 the Registered Counselor Program denied the application of registered counselor applicant Joseph A. Nagel (RC00058080). Nagel was convicted of criminal impersonation. He cannot reapply for two years.

Spokane County

In September 2007 the Medical Commission charged physician applicant Gabriel A. Campbell (MC00019824) with unprofessional conduct for allegedly failing to disclose several convictions on his application. Also he allegedly failed to complete acceptable postgraduate medical training.

In September 2007 the Licensed Social Worker Program charged licensed social worker David P. Stanzak (LW00006436) with unprofessional conduct. Stanzak allegedly made inappropriate sexual comments to patients during counseling sessions.

Thurston County

In September 2007 the Nursing Assistant Program charged nursing assistant applicant Gregory S. Benner (NA00194946) with unprofessional conduct. He was convicted of driving under the influence, reckless burning and criminal trespass. He allegedly failed to send an explanation of the convictions to the department as requested.

In September 2007 the Nursing Assistant Program charged certified nursing assistant Darcy Lockard (NC10041100) with unprofessional conduct. She allegedly took pain medications from her place of employment and tested positive for opiates in a workplace drug test.

Whatcom County

In September 2007 the Board of Pharmacy placed the license of pharmacist Christian P. Long (PH00041549) on probation for five years. He diverted drugs from his place of employment for his own use. He must comply with terms and conditions set against his license.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
