State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA • The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department» s Health Professions Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency» s Web site. Click on "Provider Credential Search" on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider» s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In October 2007 the Health Care Assistant Program suspended the credential of Leland L. Bishop (HC00141567). Bishop failed to comply with terms and conditions of a previous order.
In October 2007 Susan J. Chapman (CP00000858) entered into an agreed order with the Chemical Dependency Professional Program to voluntarily surrender her credential to practice as a chemical dependency professional. Chapman submitted documents to a court that were not authorized by her supervisor.
In October 2007 the Chemical Dependency Professional Program suspended the credential of Sherry A. George (CP00001234). George submitted an inaccurate report to a court. Her suspension is stayed if she complies with terms and conditions.
In September 2007 the Nursing Commission modified a prior order and indefinitely suspended the license of registered nurse Jennie I. Kim (RN00103741).
In October 2007 the Registered Counselor Program indefinitely suspended the credential of Erin M. Stewart (RC00057284). Stewart failed to comply with terms and conditions set against his credential.
In October 2007 the Licensed Mental Health Counselor Program suspended the credential of Michael G. Leifer (LH00005834). Leifer was convicted of reckless endangerment and failed to report this conviction as required.
In October 2007 the Nursing Assistant Program denied the application of nursing assistant applicant Diane M. Leach (NC10095896). Leach was convicted of third degree assault, hit and run and driving under the influence on two separate occasions.
In October 2007 the Massage Program suspended the credential of Jeffery A. Bray (MA00021118). Bray engaged in sexual intercourse and sexual contact with a client on numerous occasions during massage therapy treatments.
In October 2007 the Registered Counselor Program suspended the credential of Leroy Canty, Jr. (RC00039252). Canty failed to respond to department inquiries about a complaint filed against him.
In October 2007 the Health Care Assistant Program indefinitely suspended the credential of Harold J. Harding (HC00140185). Harding failed to comply with terms and conditions set against his credential.
In October 2007 the Surgical Technologist Program suspended the license of Wesley A. Hurd (ST00000281). Hurd tested positive for cocaine.
In October 2007 the Nursing Commission indefinitely suspended the license of registered nurse Janice L. Kelly (RN00162302). Kelly failed to change a patient» s dressing for several days, as ordered, and the patient's condition worsened.
In October 2007 the Chiropractic Commission suspended the license of Enayatollah Sobhani (CH00001376). Sobhani failed to comply with terms and conditions set against his license.
In October 2007 the Registered Counselor Program indefinitely suspended the credential of Terri L. Cotterell (RC00054294). Cotterell failed to comply with terms and conditions of a previous order.
In October 2007 the X-Ray Technician Program revoked the registration of Lee E. Entsminger (XT00003080). Entsminger failed to comply with terms and conditions of a previous order.
In October 2007 Yvonne Gatto entered into an agreement with the Unlicensed Practice Program to permanently cease and desist from engaging in any and all conduct constituting the practice of medicine by laser hair removal unless she becomes licensed. She also agrees to permanently cease and desist from representing herself as a radiologic technician on her business card unless she gets a credential to do so. She is not currently licensed to practice as a physician or a radiologic technician.
In October 2007 the Nursing Assistant Program indefinitely suspended the credential of Judith M. Caietti (NC10023311). Caietti was convicted of assault domestic violence.
In October 2007 the Nursing Commission summarily suspended the credential of licensed practical nurse Judith K. Middleton (LP00027494). Middleton voluntarily surrendered her credential to practice as a licensed practical nurse in Arizona.
In September 2007 the Medical Commission summarily suspended the license of David W. Shoemaker (MD00029733). Shoemaker» s medical license was indefinitely suspended by the Oregon Board of Medical Examiners for repeated acts of negligence.
Oregon: In October 2007 the Nursing Assistant Program summarily suspended the credential of nursing assistant Laura K. Hubbert (NA00169495). Hubbert» s credential to practice as a nursing assistant was revoked by the Oregon Board of Nursing after she tested positive for marijuana, failed to participate in a treatment program, refused random drug tests and failed to cooperate with the board during their investigation.
Pennsylvania: In October 2007 the Nursing Commission indefinitely suspended the license of registered nurse Wendi M. Sismondo (RN00141273). Sismondo failed to comply with terms and conditions set against her license.
Tennessee: In October 2007 the Nursing Commission indefinitely suspended the license of licensed practical nurse G. Michael Lanham (LP00056557). Lanham pled guilty to harassment and failing to obey a law enforcement officer.
Texas: In September 2007 the Nursing Commission summarily suspended the license of registered nurse Thom M. Walz (RN00165871). He voluntarily surrendered his license to practice nursing to the Texas Board of Nurse Examiners. He is prohibited from practicing as a registered nurse in Texas because of narcotic administration discrepancies.
