State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In October 2007 the Nursing Assistant Program charged nursing assistant applicant Victoria A. Cuello (NA00191993) with unprofessional conduct. She was convicted of driving under the influence on two occasions and hit and run of an unattended vehicle.

Clark County

In October 2007the Registered Counselor Program charged registered counselor Jennifer E. Martin (RC00041333) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

Cowlitz County

In October 2007 the Medical Commission entered into an agreed order with physician Mark T. Reis (MD00032312). Reis had a sexual relationship with one of his patients. He must comply with the terms and conditions set in the agreed order.

Grays Harbor County

In October 2007 the Dental Commission charged dentist Richard A. Pedersen (DE00005133) with unprofessional conduct. He allegedly provided substandard care to several patients and failed to comply with a previous order set against his license.

King County

In October 2007 the Nursing Commission charged licensed practical nurse Traci L. Albro (LP00052325) with unprofessional conduct. Albro allegedly diverted drugs from her place of employment for her own use.

In October 2007 the X-Ray Technician and Radiological Technologist Programs charged radiological technologist Amy S. Bjorn (RT00007143) with unprofessional conduct. While working as an x-ray technician she allegedly practiced outside the scope of practice of an x-ray technician by administering drugs to patients.

In October 2007 the Registered Counselor Program placed the license of registered counselor Yaffa Cohen (RC00029434) on probation for three years. She allegedly accepted $375 to conduct an adoption report for a client but never completed the report and did not refund the money. She also failed to respond to department questions about the matter. Cohen must comply with terms and conditions placed against her license.

In October 2007 the Nursing Assistant Program placed the license of nursing assistant Shahid Newad (NA00179947) on probation for three years. He touched a resident of the adult family home he operated in a manner that made the resident uncomfortable. He must comply with terms and conditions set against his license.

In October 2007 the Nursing Commission charged registered nurse Michael S. Rodriguez (RN00147746) with unprofessional conduct. He was convicted of assault.

In October 2007 the Licensed Mental Health Counselor Program charged licensed mental health counselor John E. Shephard (LH00006940) with unprofessional conduct. He allegedly made inappropriate comments to a client, hugged the client and touched the client’s hands, knees, shoulders and breasts during a counseling session.

In October 2007 the Registered Counselor and Chemical Dependency Professional Programs charged registered counselor and chemical dependency professional Lee H. Young (RC00031018 and CP00001474) with unprofessional conduct. He allegedly posted $500 bail for a client and visited the client in her home. He also allegedly represented himself as an addiction psychologist but is not licensed to practice as a psychologist.

Lewis County

In October 2007 the Nursing Assistant Program charged nursing assistant applicant Laura A. Hooper (NA00194947) with unprofessional conduct. She was convicted of unlawful issuance of bank checks or drafts and malicious mischief.

Pierce County

 In October 2007 the Nursing Assistant Program charged certified nursing assistant applicant Nannette M. Collins (NC10054187) with unprofessional conduct. She was convicted of driving with license suspended, attempting to elude a pursuing police vehicle and driving under the influence on two occasions.

 In October 2007 the Chiropractic Commission charged chiropractor James A. Devine (CH00002518) with unprofessional conduct. He allegedly kept inadequate treatment notes and billed for more adjustments than were noted in the patient’s notes.

 In October 2007 the Registered Counselor Program charged registered counselor applicant Pilar Lopez (RC00058336) with unprofessional conduct. Lopez was convicted of conspiracy to deliver a controlled substance – methamphetamine and attempted unlawful possession of a controlled substance.

In October 2007 the Nursing Assistant Program reinstated the license of nursing assistant Essie Louise Wilson (NA00034249).

Snohomish County

In October 2007 the Nursing Assistant Program reinstated the license of certified nursing assistant Judy K. Latraille (NC10019993).

Spokane County

In October 2007 the Nursing Assistant Program denied the application of nursing assistant applicant Robert B. Lavoie (CA00048262). He was convicted of harassment.

In October 2007 the Chemical Dependency Professional and Registered Counselor Programs charged chemical dependency professional and registered counselor Barbara G. Peterson (CP00003669 and RC00015250) with unprofessional conduct. She allegedly altered a client’s suicide assessment after the patient overdosed. She also allegedly had deficiencies in her records such as signing blank treatment reports, not doing six month review reports, not updating treatment plans and having incomplete intake assessments.

In October 2007 the Medical Commission entered into an agreed order with physician Nathan P. Stime (MD00010797). He failed to properly evaluate and treat a patient and failed to maintain thorough records of treatment and evaluation resulting in delayed diagnosis. Stime must comply with terms and conditions set against his license.

Thurston County

In October 2007 the Registered Counselor Program charged registered counselor Mary T. Giles (RC00048148) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

Yakima County

In October 2007 the Nursing Assistant Program charged nursing assistant Jenny M. Dove (NA00140029) with unprofessional conduct. Dove was convicted of assault of a child – domestic violence and she allegedly failed to respond to department inquiries regarding the conviction. She is also known as Jennine Marie Dove.

In October 2007 the Nursing Assistant Program denied the reactivation application of nursing assistant Raquel Ledesma (NA00123505). She was convicted of theft-unlawful taking, threats to do harm and unlawful issuance of bank checks or drafts on two different occasions. Ledesma cannot reapply for five years.

In October 2007 the Nursing Assistant Program denied the reactivation applicant of certified nursing assistant Jonni A. Wiltfong (NC10076898). She was convicted of drive-by shooting. Wiltfong cannot reapply for seven years.

Out of State

Oregon:  In October 2007 the Massage Program denied the application of massage practitioner applicant Anthony F. Dinsmore (CA00009659). Dinsmore was convicted of theft. He cannot reapply for five years.

Texas:  In October 2007 the Medical Commission charged physician applicant Aliyah Qureshi (MC00016036) with not meeting the requirements for license. An examination allegedly found many deficiencies in her knowledge and ability to diagnose and treat patients.

Note to Editors:  Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
