State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clallam County

In November 2007 the Nursing Assistant Program charged nursing assistant applicant Donna A. Reedy (NA00194605) with unprofessional conduct. She was found guilty of theft in Oregon.

In October 2007 the Nursing Commission charged registered nurse and advanced registered nurse practitioner Deborah A. Stokke (RN00051571 and AP30001238) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

Grant County

In October 2007 the Registered Counselor and Chemical Dependency Professional Programs charged registered counselor and chemical dependency professional Estela Yarrito (RC00029261 and CP00003751) with unprofessional conduct. She allegedly allowed a client’s clean urinalysis specimen to be substituted for another client’s dirty urinalysis specimen and falsified the results of other urine screenings.

Island County

In November 2007 the Nursing Assistant Program charged nursing assistant applicant Brandy L. Shuler (NA00190100) with unprofessional conduct. She was convicted of theft and entered into deferred prosecution for driving under the influence and minor in possession of alcohol. She allegedly failed to disclose the conviction and deferred prosecutions on her application.

King County

In November 2007 the Registered Counselor Program charged registered counselor applicant Sarahlynne S. Cieslikowski (RC00058161) with unprofessional conduct. She was convicted of assault of a child. She is also known as Sarah L. Cieslikowski.

In October 2007 the Board of Pharmacy placed the license of pharmacist Sohrab Daneshfar (PH00021224) on probation for one year. Daneshfar was a manager of a pharmacy and had numerous non-English labeled over the counter drugs offered for sale, accepted prescriptions by fax from unlicensed branches and filled the prescriptions for distribution. She must comply with the terms and conditions set against her license.

In November 2007 the Nursing Assistant Program charged nursing assistant applicant Destinie M. Erskine (NA00191933) with unprofessional conduct. She was convicted of theft and unlawful issuance of bank checks. During the departments investigation of the convictions she allegedly did not cooperate with the investigators in a timely or capable manner.

In October 2007 the Registered Counselor Program charged registered counselor Rochelle A. Forsness (RC00056603) with unprofessional conduct. She failed to comply with previous terms and conditions set against her license.

In November 2007 the Health Care Assistant Program denied the application of health care assistant applicant Cheri A. Murphy (CA00012340). She called in and picked up approximately 20 prescriptions of legend drugs in a family members name. The prescriptions were not ordered by a nurse or a physician. She cannot reapply for three years.

In November 2007 the Nursing Commission charged licensed practical nurse Shenee S. Poe (LP00053333) with unprofessional conduct. She allegedly wrote prescriptions for her brother and signed the name of a physician who did not authorize the prescriptions.

In October 2007 the Massage Program denied the application of massage therapist applicant Issaac L. Rakestraw (CA00008767). He was convicted of shoplifting, possession of a dangerous weapon, assault, disorderly conduct on three different occasions, minor in possession of and/or consuming alcohol, obstructing a public servant, possession of drug paraphernalia and assault with a deadly weapon.

In October 2007 the Registered Counselor Program granted the application of registered counselor applicant Antony L. Xczar (RC00059839) and placed his license on probation for two years. He was convicted of assault. Xczar must comply with the terms and conditions set against his license. He is also known as Antony L. Carr.

Kitsap County

In October 2007 the Medical Commission reprimanded physician Alfred Aflatooni (MD00015674) and placed his license on probation for an indefinite period of time. He had physicians under the Physician Visa Waiver Program sign promissory notes effectively reducing their pay and violating the contract filed with the department. He must comply with the terms and conditions set against his license.

In October 2007 the Registered Counselor Program charged registered counselor applicant Dawn Wilson (RC00058553) with unprofessional conduct. She was convicted of assault.

Pierce County

In October 2007 the Nursing Assistant Program charged certified nursing assistant Shebuel A. Bel (NC10075945) with unprofessional conduct. Bel was convicted of assault.

In October 2007 the Medical Commission censured physician Robert E. Billings (MD00013280). He wrote prescriptions for himself and named other physicians on the prescriptions without their permission. He must comply with the terms and conditions set against his license.

In October 2007 the Board of Pharmacy granted the application of pharmacy assistant applicant Molly Komiti (CA00066679) and placed her license on probation for five years. She was convicted of theft but the charges were dropped after she complied with the terms and conditions of the deferred sentence. She failed to disclose the conviction and dismissal on her application. Komiti must comply with the terms and conditions set against her license.

Thurston County

In October 2007 the Dental Commission amended the statement of charges against dentist Sherilyn V. Malloy (DE00006693). She allegedly failed to properly prepare a tooth for restoration and failed to adequately document the care given to patients, including conscious sedation. She is not licensed to administer conscious sedation.

In October 2007 the Nursing Assistant Program charged nursing assistant Remedios S. Oliveros (NA00122321) with unprofessional conduct. Her adult family home license was revoked. She allegedly yelled at a resident, failed to ensure residents got their medications and failed to follow doctor’s orders.

Whatcom County

In October 2007 the Medical Commission entered into an agreed order with physician Kenneth W. Kloehn (MD00011623). A patient was injured after Kloehn allowed more oxygen to be administered to a patient than the patient needed and failed to use a fire suppressing technique. He must comply with the terms and conditions set against his license.

Out of State

Canada:  In October 2007 the Medical Commission entered into an agreed order with physician Yair Karas (MD00014040). Karas entered into an agreement with the College of Physicians and Surgeons of Ontario in which he agreed not to perform radial keratotomy. He must comply with the terms and conditions set against his license.

Missouri:  In October 2007 the Medical Commission charged physician David S. Debyle (MD00028687) with unprofessional conduct. His Missouri license is on probation for allegedly diverting drugs and resigning from hospital privileges while under investigation.

Ohio:  In October 2007 the Medical Commission charged physician David C. Blocker (MD00027013) with unprofessional conduct. His Ohio license is restricted based on restrictions imposed by the Air Force and based on the recommendations from an assessment of Dr. Blocker’s skills. His also entered into an order with the Kentucky Board of Medical Licensure based on the findings in Ohio.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
