OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In November 2007 the Dispensing Optician Program reprimanded dispensing optician apprentice Linda L. Cohen (DT10005710). Cohen failed to send a written explanation to the department regarding her previous work activities.
In November 2007 the Nursing Assistant Program granted the application of nursing assistant applicant Angela Uruo (NA00199800) and placed her license on probation for three years. She was convicted of harassment. She must comply with terms and conditions set against her license.
In November 2007 the Nursing Commission charged licensed practical nurse Helen Henrikson (LP00044172) with unprofessional conduct. Allegations include failing to administer medication to patients based on physician orders and mistakenly hooking up a feeding tube to the supra-pubic catheter instead of the gastrostomy tube, resulting in the patient being sent to the emergency room for surgery.
In November 2007 the Nursing Assistant Program charged certified nursing assistant applicant Danielle A. Ackerlund (NC10099104) with unprofessional conduct. She was convicted of passing bad checks and forgery.
In October 2007 the Board of Psychology charged psychologist Dennis R. Allen (PY00002454) with unprofessional conduct. During parenting evaluations related to divorces, he allegedly appeared biased, spoke inappropriately to the children of the clients, misrepresented consultation with another psychologist, failed to maintain records and crossed professional boundaries after the completion of the evaluation.
In November 2007 the Dental Commission entered into an agreed order with dentist Kam Derakshani (DE00007712). He treated a patient below the standard of care while performing crown and bridgework. He must comply with the terms and conditions set against his license.
In November 2007 the Nursing Assistant Program corrected an amended statement of charges against nursing assistant Elizabeth M. Gichuki (NC10084369). She allegedly left a developmentally disabled adult patient alone and locked in a vehicle for an hour and pled guilty to reckless endangerment.
In November 2007 the Medical Commission corrected the statement of charges against physician Shahm Martini (MD00042323). Allegations include failing to end treatment with a patient who was making sexual advances towards him, engaging in sexual activities with the patient and discussing the patient’s treatment and diagnosis with another patient.
In October 2007 the Medical Commission charged physician Kinne D. McCabe (MD00017145) with unprofessional conduct. Allegations include failing to be present at a study he was supposed to supervise, failing to sign consent forms before the study took place and failing to examine any of the patient’s in the study he agreed to supervise.
In November 2007 the Health Care Assistant Program charged health care assistant Rheycel S. Orante (HC00150116) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.
In November 2007 the Nursing Commission charged licensed practical nurse Wendy E. Arneson (LP00045192) with unprofessional conduct. Allegations include charting she changed wound dressings on patients when she had not, removing medication from a supply cabinet without permission and mistakenly administering the wrong medication to the wrong patients.
In November 2007 the Nursing Assistant Program charged nursing assistant Malissa M. Cunningham (NA00181248) with unprofessional conduct. She was convicted of conspiracy to possess a controlled substance with intent to deliver and unlawful possession of a controlled substance.
In November 2007 the Nursing Assistant Program charged nursing assistant Lyndell F. James (NC10041285) with unprofessional conduct. James was convicted of theft and allegedly failed to respond to department inquiries regarding the conviction. She is also known as Lyndell F. Hines.
In November 2007 the Nursing Assistant Program entered into an agreed order with certified nursing assistant reactivation applicant Lori A. Moore (NC10021443). She was convicted of driving under the influence and reckless endangerment. She was given a credential but must comply with the terms and conditions set against her license.
In November 2007 the Nursing Commission charged licensed practical nurse Lorilyn M. Schrader (LP00026541) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.
In November 2007 the Nursing Commission charged licensed practical nurse Carol G. Johnson (LP00021874) with unprofessional conduct. She allegedly placed a pillow case over the head of an elderly dementia patient to prevent the patient from spitting at or biting her.
In October 2007 the Dental Commission charged Dentist William M. Petersen (DE00003491) with unprofessional conduct. Allegations include failing to provide or document a treatment plan, failing to do pre-operative tests for diagnosis, failing to obtain consent for treatment to be performed and failing take or record taking patients blood pressure before administering anesthetics.
In November 2007 the Nursing Assistant Program charged nursing assistant Michael J. Armijo (NA00181882) with unprofessional conduct. He was convicted of malicious mischief – domestic violence on two occasions and assault – domestic violence on two occasions.
In November 2007 the Nursing Commission charged licensed practical nurse Stefanie R. Ambuehl (LP00058765) with unprofessional conduct. Ambuehl’s license to practice as a registered nursing assistant was revoked and she not disclose the revocation when she applied for her licensed practical nurse license. She is also known as Stefanie R. Millspaugh.
In November 2007 the Nursing Commission charged licensed practical nurse Dennis Sean Pratt (LP00055862) with unprofessional conduct. He allegedly diverted drugs from his place of employment for his own use, improperly delegated tasks to nursing assistants and failed to respond to department inquiries regarding the allegations.
In November 2007 the Medical Commission charged physician Roger E. Sharf (MD00029218) with unprofessional conduct. He allegedly entered into a sexual relationship with a patient.
In November 2007 the Nursing Commission amended the statement of charges against licensed practical nurse Brenda Kay Holden (LP00050415). She allegedly failed to comply with previous terms and conditions set against her license.
Beirut, Lebanon: In November 2007 the Medical Commission corrected the statement of charges against physician Kamal F. Aboulhosn (MD00022863) with unprofessional conduct. Allegations include making inappropriate sexual comments to patients, touching patients inappropriately and performing unsatisfactory breast augmentations that required additional surgery.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
