OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Professions Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency’s Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In October 2007 the Physical Therapy Board suspended the license of Rafat Shirinzadeh (PT00007740) for 36 months. He engaged in sexual misconduct and provided substandard care to several patients. Shirinzadeh can request reinstatement if he complies with terms and conditions.
In October 2007 the Nursing Assistant Program revoked the credential of Gloria Davalos (NC10012476). Davalos was convicted of conspiracy to deliver methamphetamine and possession of cocaine. She is also known as Gloria Davalos-Cabrillas.
In November 2007 the Nursing Assistant Program suspended the credential of Allison R. Seamands (NA00183924). Seamands failed to respond to department inquiries regarding a complaint.
In November 2007 the Physical Therapy Board summarily suspended the license of Eli N. Lambert (PT00008627). Lambert’s license to practice physical therapy was revoked by the Hawaii Board of Physical Therapy.
In October 2007 the Registered Counselor Program revoked the credential of Ann E. Perry (RC00041319). Perry had an intimate relationship and lived with a patient.
In November 2007 the Registered Counselor and Licensed Marriage and Family Therapist Programs revoked the credentials of Flor Mari Crisostomo (LF00002586 and RC00029110). Crisostomo had sexual contact with a minor client.
In November 2007 the Nursing Assistant Program revoked the credential of Darlan M. Davis (NA00165945). Davis was convicted of third degree assault and failed to respond to department inquiries regarding the conviction. She is also known as Tina A. Watson.
In October 2007 Calvin Lewis entered into an agreed order with the Unlicensed Practice Program to permanently cease and desist from engaging in any and all conduct constituting the practice of nursing assistant unless Lewis obtains the requisite health care credential. He worked as a nursing assistant after his credential was revoked.
In October 2007 the Nursing Assistant Program revoked the credential of nursing assistant Masoud A. Mohamed (NC10074829). Mohamed was convicted of violation of a no contact order and domestic violence misdemeanor violation of a court order.
In October 2007 Julie Wrimer entered into an agreed order with the Unlicensed Practice Program to permanently cease and desist from engaging in any and all conduct constituting the practice of massage unless Weimer has first obtained a license or otherwise meets an exemption.
In October 2007 the Chiropractic Commission indefinitely suspended the license of Alan Weiss (CH00003286). Weiss abandoned patients with no recommendations towards other treatment providers. He also failed to refund patients for payments made in advance.
In November 2007 the X-Ray Technician Program suspended the credential of Jennifer L. Rone (XT00004362). Rone took a legend drug from her employer and injected herself with it on multiple occasions without a prescription. She filled 40 forged prescriptions for other drugs.
In November 2007 the Nursing Assistant Program indefinitely suspended the credential of Hiedi A. Siel (NC10083265). Siel ingested controlled substances prior to work at an adult care home.
In October 2007 the Health Care Assistant Program suspended the credential of Kimberly Blevins (HC00138925). Blevins forged a doctor’s letter and pled guilty to first degree theft. She cannot reapply for three years.
In November 2007 the Pharmacy Board indefinitely suspended the license of pharmacist Anthony R. Burns (PH00015410). Burns failed to comply with terms set against his license.
In October 2007 the Nursing Assistant Program revoked the credential of nursing assistant Shanna A. Carter (NC10064547). Carter was convicted of third degree assault.
In October 2007 the Denturist Program suspended the credential of denturist Thomas Lee Goebel (DN00000297) for 30 months. Goebel can reapply if he complies with terms set against his license. He was convicted of second degree assault.
In October 2007 the Nursing Commission indefinitely suspended the license of licensed practical nurse Christine R. Riley (LP00055350). Riley failed to comply with terms and conditions set against her license.
In November 2007 the Dental Commission withdrew charges against Jae Hong (DE00008227).
In November 2007 the Dental Commission withdrew charges against Sang C. Kim (DE00009494).
In October 2007 the Chiropractic Commission granted the license for applicant Douglas Gainor (CA00002955) with set terms and conditions, but immediately suspended it. The suspension will be stayed if Gainor meets certain conditions. His California license is on probation.
In October 2007 the Unlicensed Practice Program issued a final order to Alexis Noel to permanently cease and desist from engaging in any and all conduct constituting the practice of licensed practical nurse in the state of Washington unless Noel has first obtained the appropriate credential or otherwise meets and exemption. She applied for a job using a forged credential.
In October 2007 the Nursing Home Administrators suspended the license of Helen June Nelson (NH00000991) for five years. The suspension is stayed, but subject to terms and conditions. Nelson borrowed money and gained powers of attorney from vulnerable residents.
In November 2007 Ronald D. Graham (LH00006130 and CP00001598) agreed to surrender his credentials to the Licensed Mental Health Counselor and Chemical Dependency Programs. Graham did not protect confidential health care records, fraternized with a client and failed to disclose a conviction on his application.
California: In October 2007 the Medical Commission modified an order against physician Craig E. Swanson (MD00027758). His license is placed on stayed suspension and he must comply with terms and conditions.
Canada: In October 2007 the Nursing Commission indefinitely suspended the license of registered nurse Greg S. Neufeldt (RN00123876). Neufeldt signed a consent agreement equivalent to a suspension from practicing as a registered nurse with the College of Registered Nurses of British Columbia.
Arizona: In October 2007 the Nursing Assistant Program revoked the credential of Angela R. Vicari (NC10086208). Vicari’s credential to practice as a certified nursing assistant in Arizona was revoked for failure to comply with a previous order.
Oregon: In October 2007 Lawrence R. Vidrine (MD00022122) agreed to surrender his license to the Medical Commission, in lieu of other sanctions, to practice as a physician in the state of Washington. Vidrine provided substandard care to a patient.
Oregon: In October 2007 the Pharmacy Board summarily suspended the credential of pharmacist Charles L. Peckerman (PH00022492). Peckerman had restrictions placed on his pharmacist licenses in Oregon and California.
Texas: In October 2007 the Nursing Commission revoked the license of registered nurse Thom M. Walz (RN00165871). Walz voluntarily surrendered his credential to practice as a registered nurse to the Texas Board of Nurse Examiners when charged with withdrawing narcotics for patients without following policy and procedure for wastage of the medications, among other discrepancies.
